High Fidelity Wraparound Services - FamilyWise Services

Provides services that are intended to prevent out of home placement for youth who are being considered for placement in correctional facilities or residential treatment facilities. Helps create a plan for the family to continue to receive support from the community after social services end. Program staff works with the family and team members involved in the plan to follow up on goals. This process continues until the family feels stable and supported enough to coordinate the wraparound process on its own.

High Fidelity Wraparound Services - Volunteers of America Minnesota and Wisconsin

Provides supportive services for youth and their families with complex mental health needs by focusing on their strengths and support system including a team-based care coordination process that honors and builds on family culture. Wraparound Care Coordinators are trained Children's Targeted Mental Health Case Managers and certified as Wraparound providers. They help families develop their plan, build their team of informal and formal supports, and facilitate the process. Certified Family Peer Specialists provide support to families encountering children's mental health challenges and partner with Care Coordinators to support families.

Restorative Justice Circle Program - Southwest Health and Human Services

Circle brings people together and provides a process for organizing effective group communication, relationship building, decision-making, and conflict resolution. Restorative practices help create a community that honors the importance of relationships amongst all members in the community and repair relationships when harm has been caused. - Circle of Hope Circle of support for those working towards finding and maintaining ongoing recovery from substance abuse. - Family & Community Circle Creates support networks for youth transitioning home from placement and works with youth and families with a variety of familial matters that may be leading to placement or family conflict. - Juvenile Circle Sentencing An alternative response to a crime that promotes healing and safety for everyone. Included in the Circle process are those harmed by crime/negative behavior, those who commit crime, and the community. - Other Circles As needed (i.e. schools, parenting, foster parent, etc.)
