Delivers personalized, in-home medical and therapy care to help people recover from illness, surgery, or manage chronic conditions. Services include: - Home health aide support (assistance with daily activities and personal care when needed) - Medical social work/support: Helps patients with emotional, social, or resource needs, and coordinates care plans with families and doctors. - Occupational therapy: Helps with daily living tasks like dressing, bathing, and adapting the home environment for safety - Physical therapy: Rehabilitation after surgery or injury, mobility, and strength restoration - Skilled nursing: Medication management, chronic disease monitoring, wound care, IV/infusion therapy, and vital-sign checks - Specialized care programs for chronic conditions or specific needs - Speech therapy: Assistance with speech, communication, and swallowing difficulties, often after a stroke or neurological issues
