Mental Health Counseling - African American Child Wellness Institute

Academic Wellness and Success Program: School-linked program designed to increase the availability of mental health services to children in pre-K - 12th grade of African descent who are uninsured or underinsured in their homes, schools, communities, and in the African American Child Wellness Institute offices through the use of face-to-face and telehealth platforms. Provides approximately 8-10 hours of direct mental health services including mental health screenings, diagnostic assessments, and psychotherapy treatment including individual, group, and family sessions through the use of cultural specific providers. Counseling, Outreach, and Public Education Campaign: Addresses COVID-19 prevention and vaccine hesitance within the African-American community. Services include a 24-hour Community Mental Health and COVID-19 hotline, individual, and group crisis counseling (within multi-family settings and outside of the home), individual and group outreach services, as well as brief educational or supportive contact, public education, assessment, and referral to community resources. Healing Circle, The: Provides a sacred space where community-based challenges are addressed in an orderly manner. Life and death issues such as COVID-19, community violence, and systemic racism are discussed. Provided in collaboration with the Conversations with Al McFarlane program which airs of KFAI Radio Station and is streamed live on Facebook and YouTube. Mura Moms: Pre-meditated pregnancy and motherhood project which provides alternative methods of engaging African American women in the process of examining how their stress, depression, anxiety, and substance use are harmful to their own well-being as well as that of their baby, and to allow them to learn culturally appropriate stress reduction techniques and coping mechanisms in supportive environments. Overall goal is to improve birth outcomes among low-income African American women struggling with chemical use and/or mental health issues. To accomplish this, both parents are invited to develop knowledge, partnerships, skills, and behaviors that promote wellness, safety, and respect in their homes and in the community. Provides intensive support including intake, education, mentorship, and wellness coaching services to up to 15 women per class. Project Mura: Pre-meditated parenting BootCamp which provides three intensive BootCamp trainings annually for parents who are raising uninsured or underinsured children of African descent ages 0 - 18. Services include diagnostic interviews, individual psychotherapy, family therapy, group therapy, crisis intervention, and other services. Clinicians provide cultural consultations and care coordination services with other mental health providers, schools, medical personnel (such as nurses, primary care providers, personal care attendants, pediatricians, and psychiatrists) as well as other entities such as religious and social service organizations to provide multi-systemic support. Individual wellness coaches facilitate the development of family wellness plans for each program participant.

Health and Fitness - YMCA of the North

Group Exercise Classes: Members can attend a variety of free fitness classes, including cardio and strength, dance, strength, yoga and pilates, ForeverWell, Kids and Family, and water exercise. Note: Not all classes are offered at all locations. Personal Training and Assessments: Rates for members and non-members. Members can receive a free one-on-one 60-minute assessment session with a certified personal trainer. Personal training sessions are available virtually via Zoom (included in membership or a monthly fee). Small Group Training Programs: - Provides opportunities for small group training and flex training. Available classes vary by location. Rates for members and non-members. - Wellness 180 provides a series of lessons that will help educate participants in and out of a classroom setting on how gradual lifestyle changes can positively impact their health. Rates for members and non-members. - Wellbeats is for kids ages 3 to 14 who are participants of Kids Stuff or child care (must be a member). Kids Stuff provides free drop-off child care while parents/caregivers work out (must be a member). Elder Care: Provides specific programs and services to support older adults age 55 and older, including in-person activities and free classes. - ForeverWell: Provides members with wellness activities for older adults age 55 and older including virtually online or in-person options. Must be a member for the virtual online activities. Also provides wellness checks for senior ForeverWell members. The George Wellbeing programs offer guidance, virtually or one-on-one in a group setting, with a board-certified coach, to help participants navigate healthy choices. Adult Sports Leagues: Provides team, doubles, and individuals sport activities. Opportunities for members and non-members. Kids Sports: Provides year-round activities and program for kids of all ages and skill levels, including youth sports leagues and classes, summer sports, Homeschool Physical Education, and adult sports leagues. Members and non-members are welcome to participate. Offers youth with disabilities age 8 and up opportunities to participate in the Blazing Stars Special Olympics Team. The Young Athletes program introduces children ages 2 to 7 with and without intellectual disabilities to the world of sports. Orthology: Provides physical therapy and chiropractic services Kids and Families: Provides exercises classes, adventure racing, free child care while members work out, Parents Night Out provides child care for an evening out, family friendly fitness centers, and youth and family fitness cards that outlines an entire workout from warm-ups to cool down stretches. Journey to Freedom: Provides an 8 week session where participants meet in small-group discussions, read, reflect, find support, and heal. The goal of the program is to build community and support successful life change in a safe place. Specialty series include Freedom from Body Image and Food Issues, Freedom from Low Self-Esteem, Freedom from the Pain of Grief and Disappointment, and Freedom from Fear. Condition Specific Programs: Include tailored health and fitness programs for people with health conditions including Parkinson's disease and pre-diabetes. Water Safety Program: Provides reduced cost water safety instruction for kids ages 5 - 14 and swimming lessons for ages 6 months to adult. Offers parent and child, school-age children, preschool children, and teen and adult swimming lessons and water safety instruction. Member and non-member rates available. Provides trainings and certifications for lifeguard training, CPR and first aid, babysitting safety, safety training for coaches and instructor level training. Classes are offered in a traditional classroom and blended where lessons are online and skills are practiced in class.

Journey Transitional Living Program Program to establish independent living arrangements and skills training for homeless youth Red Path Program Program created to assist youth leaving placements in foster care, residential treatment, group homes, or other residential care facilities with living skills classes, transition planning, and case management. Includes: - Budgeting and finance - Juvenile justice - Healthy relationships - First impressions - Renter's education - Sexual health - Diversity - Leadership and community development Emergency Youth Hotline Program providing temporary shelter for runaway or homeless youth, free consultation and counseling as well as support, therapy sessions, and assistance communicating with family to resolve conflicts. Education Support Services - YMCA of the North

Career Pathways: Provides youth ages 14 - 24 who have faced systemic barriers to employment the opportunity to be introduced into the workforce by getting an internship at the YMCA. Beacons: Provides after-school and summer care for young people in grades K - 12 by partnering with Twin Cities area schools to provide quality educational, recreational, and leadership development activities and eliminate barriers to participation. Center for Youth Voice: Provides programs and services that help young people understand our system of government and how to effectively use their voice to take action on issues that matter to them. Teen Tech Center: Provides a technology-driven, interactive space for young people ages 12 - 24 to drop in, be creative, and develop new skills for free. Does not require a YMCA membership. Must be a member of the Teen Tech Center.

Offers educational programs such as: - Best Buy Teen Tech Center: A drop-in space for teens ages 14 - 18 to be creative and develop new skills. The center helps youth expand their interests and discover career paths through technology-based projects in programming, film-making, music production, graphic design, and much more. - Citizenship Classes: Prepare students to pass the United States Citizenship exam successfully. Classes are taught in English only. The class topics include the process of becoming a citizen, United States history and government, interview preparation and practice, study materials, and other resources. - English Language Classes (ESL): Classes open to anyone who would like to improve their English skills. Students learn how to read, write, speak, and understand the English language while also gaining essential life skills that will better prepare them to navigate life in the United States. Classes are offered online on Zoom. - GED Preparation Classes: Help students prepare to take the GED (High School Equivalent) exam. Classes cover all testing subjects, including math, social studies, science, and language arts (grammar and writing). Ongoing Support and Planning for Stability - Dignity Center

Trained volunteer advocates work one-to-one with individuals to help them achieve stability and greater self-sufficiency through services that include: - Accountable relationships - Referrals to community programs and services - Self-development opportunities - Teaching the skills needed for self-sufficiency such as financial management, interviewing, organization, gaining and maintaining stable housing, social skills, self-care, making appointments, and self-advocacy ?NOTE: Participants are expected to make and keep appointments with their advocate at least once a month. Resources are available to support actions taken towards stability goals.

Residential Treatment Cottage - North Homes Children and Family Services

A safe place where counselors, therapists, and doctors create a therapeutic living environment to help students who reside in this setting gain the skills they need to succeed in their home, community, and school. Residential care and treatment for boys and girls ages 10-17 in a non-secure setting. There are 20 beds in the Cottage. Youth are referred to the Cottage by County Social Workers, County Probation Officers, Tribal Social Workers, Parents, self-referrals, or the courts. On-site educational programming is provided by Independent School District 318. Each student will earn credit for school attendance during placement at the North Homes Cottage. The following areas are addressed in each resident's plan: - Social/Interpersonal Skills - Emotional Control - Vocational Skills - Independent Living Skills - DBT and skills Students in the Residential Program participate in various program activities and receive services including, but not limited to: - Daily therapeutic/psycho-educational group discussions covering such topics as communication skills, emotions, anger management, gender issues, CD information, stress management, depression and conflict resolution - Individual counseling - Psychiatric evaluation and monitoring - Structured daily recreational/leisure activities - Structured daily schedule addressing daily living skill development - Cultural education, activities and advocacy - Community professional mental health services - On site school programming - Coordination with community resources and agencies - On site health screening and care - Spirituality services Treatment will have individual goals for everyone to complete. Early Childhood Family Education - ECFE - Cedar Mountain Public School District 2754

ECFE is a parenting education program for all Minnesota families with children between the ages of birth to Kindergarten entrance. Learning opportunities through ECFE may occur in a variety of settings, such as the families' homes, district or community sites, and online parenting education and support. Programming and education services often include parents/caregivers and children but may just include parents/caregivers. Works to strengthen families and engage the ability of all parents to provide an environment that supports the healthy development of their child. Community Health Programs - Mille Lacs County Community and Veterans Services - Human Services Programs

Provides community health programs such as: - Child and Teen Checkup Outreach: Helps families find a doctor, dentist, psychologist, and transportation - Disease and Prevention Control: Monitor and investigate potential disease processes through adult/child immunizations, communicable disease prevention programs, emergency preparedness plans, surveillance of the county's environment, and tuberculosis screening - Environmental Health: Help protect the residents from environmental health hazards through programs to reduce exposure to health risks, such as food and waterborne diseases, radiation, occupational health hazards, and public health nuisances. Provides radon, lead, and well water education. - Follow-Along Program: Provides parents with information about their child's development during infancy and early childhood through mailing questionnaires - Health Promotion: Provides presentations and/or materials on health topics for small and large groups. Topics may include chemical and tobacco use prevention, family health, mental health, nutrition, and physical activity - Public Health Preparedness for Emergencies: Funded by the Center for Disease Control and Prevention in partnership with local, state, and territorial public health departments to provide assistance to public health departments across the nation. Helps health departments strengthen abilities to effectively respond to a range of public health threats, including infectious diseases, natural disasters, and biological, chemical, nuclear, and radiological events.

Support Group - Breaking Free

Program features weekly prostitution support groups virtually. The 14-week group has a curriculum created and developed by survivors of the sex work life. After graduating, all participants are encouraged to attend an aftercare alumni group. Additional groups and services are available for relapse prevention, life skills/job skills, a speakers/writers bureau, psychotherapy by licensed practitioners weekly, street and community outreach, and a drop-in center available 40 hours a week.

Housing and Community Support Programs - Rise, Inc.

Offers person-centered support services to individuals living with a disability or mental health condition. Services provided: - Case management to assess gaps in services to develop a goal plan to resolve the individual's housing crisis and prevent them from becoming homeless - Develop life skills including self-sufficiency planning, establish independence, and housing skills - Help individuals secure and maintain stable housing. Services include house searching, application assistance, budgeting, landlord communication, and ongoing stability planning. - Meeting with staff for information about housing resources for rapid re-housing, transitional housing, permanent supportive housing, and eligibility. Provides referrals and resources to programs as needed.

35 Day Evaluation Program - North Homes Children and Family Services

The North Homes Cottage offers a comprehensive 35 Day Evaluation to provide referral sources, children, and their families' appropriate recommendations to meet the client's needs. The Assessment Unit is staffed with trained youth counselors and Mental Health Practitioners who interact daily with clients and provide behavioral observations. The client will remain in the Assessment Unit for 35 days and participate in educational programming through independent School District 318, recreational, leisure, cultural, and spiritual activities, therapeutic groups, art therapy, medication education, eligibility for becoming a positive peer, visitation with family, etc. - 24-hour behavior observation and documentation by trained staff, Licensed Independent Clinical Social Workers. A comprehensive assessment, which identifies child and family issues, and historical information is conducted. The assessment team then recommends ongoing services that will best meet the present and future needs of the child and their family. The North Homes Cottage Assessment Program consists of components including: - Behavioral Assessments - Psychiatric Evaluation - Culture Assessment - Recreational Assessment - Academic Assessment - Family Assessment - Family History - Health Screening/Physical Examination - Substance Use Assessments The evaluations will be completed in 35 days, after which an assessment staffing will be held with members of the Treatment Team to discuss findings and recommendations.
