Behavioral and Residential Services - Saint Francis Health Services - Minnesota

Provides homes and services to people with developmental and delayed disabilities, mental illness, traumatic brain injuries, and other health-related needs wherever identified in communities throughout Minnesota. Individuals can participate in day placement, day treatment, and vocational readiness on a regular basis. Adult foster care - Provides food, lodging, and supervision services to a person with disabilities, mental illness, and brain injury. These services can be short term or long term. 62 residential program sites/ 261 beds. 12 sites offer a 5th bed or apartment with services, for 25 total beds. 4 AFC beds. Children's foster care - A 24-hour substitute care for children placed away from their parents or guardians and for whom the state agency has placement and care responsibility. 4 residential program sites/8 beds. Community-based services - Community support programs - Independent living services - In-home services, including training for of the person and family members to increase their capabilities to care for and maintain the person in their home - Personal care assistant - Respite care services - Supported employment - Supported living services Intensive residential treatment - Short term for up to 90 days. 10 bed facilities at Brentwood (Marshall) and Fresh Start (Duluth). Brentwood has 1 of the 10 beds as a swing bed (crisis and IRT bed). Fresh Start serves an all-female population. 20 total IRT beds. Intellectual disabilities - Provides an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID). Individual must have a developmental disability or related condition by their 22nd birthday and requires a 24-hour plan of care. 10 facilities, 58 beds. 5 beds at Arlington in Fergus Falls and Rollingwood in North Mankato. 6 beds at Prairiewood, Rosewood, and Scenicwood in Alexandria; Lakewood in Spicer; Northwood in Red Lake Falls; Eastwood and Westwood in Redwood Falls; and Ravenwood in Morris.

Family Services - Trillium Services

Offers individualized supports for people with disabilities and their families, both in and outside the home. Services are person-centered and designed to empower individuals while providing meaningful support to families. Services include: - Consulting Services: Designated Coordinator and Designated Manager consulting for family foster care and small providers, ensuring compliance with MN Statute 245D. ?- In-Home Support Services (IHS): 1:1 skill-building, teaching, and training in the home, including community outings and activities. - Respite Care: Temporary in-home or out-of-home relief for families and caregivers, ranging from a few hours to extended stays. - Skill Development Activities: Support in areas such as cooking, fitness, health, hygiene, medication management, money handling, relationship building, safety, and social participation.

Senior Programs - Keystone Community Services

Active Seniors: Provides adults 55+ with a variety of diverse activities such as exercise, wellness, and social/recreation designed to support independence Care Consultation: Keystone Care Consultants help seniors navigate their health, economic, and social needs. Care managers meet with individuals in their homes or at one of the community centers to conduct an assessment, gather input from caregivers, and create goals. Once the assessment is complete, services may include: setting up rides to a social program, researching housing options, making a referral to an agency that helps with grocery shopping and/or cleaning, providing caregiver support, and accompanying seniors to doctor visits. Grocery Delivery: Provides free grocery delivery for low-income seniors and adults with disabilities. Those who qualify can receive monthly deliveries. Delivery can include meat, dairy, fresh produce, and shelf-stable items. In each delivery, individuals will receive an average of 25 pounds of food per person in the household. Accommodations are based on customer choice models such as vegetarian, halal, etc. depending on what is available. Deliveries will be dropped off between 12:15 pm and 1:30 pm Tuesday - Thursday by a volunteer. In-Home Help: Social workers coordinate with Recover Health to provide seniors with in-home nursing care, in-home therapies, and home health aides to assist with daily care. ?Tax Preparation Assistance: Provides tax services in coordination with AARP to seniors. Companion/Night Supervision Services: Services that help an individual work toward a therapeutic or community integration goal. Night supervision is an overnight assistance and monitoring provided in the person's own home, which includes assistance with Activities of Daily Living (ADL's), reinforcing independent living skills, etc. Homemaking/Home Management Services: Provide assistance to individuals who are unable to perform day-to-day household duties and have no one available to assist them. Services include light housekeeping, laundry, limited personal care, grocery shopping, meal preparation, etc. Individual Community Living Support (ICLS): Provides assistance and support to clients who need reminders, cues, intermittent/moderate supervision, or physical assistance to remain in their own home. Services include assistance with active cognitive support, Activities of Daily Living (ADLs), household management, health, safety, and wellness, and community engagement. Personal Care Assistant (PCA): Performs care services to clients unable to live independently in the community without assistance. Services include assistance with Activities of Daily Living (ADLs), grooming/personal hygiene, light cleaning, cooking, etc. They also engage clients in activities such as reading, talking, community support, etc. Personal Support Services/Individualized Home Supports Without Training: Provided in the individual's home to achieve their full potential, to increase independence, and to meet community inclusion goals that are important and based on their need. Established to assist with Activities of Daily Living (ADLs), participation in community activities, etc. Respite Care: Short-term care services provided due to the absence or need for relief of the family member(s) or primary caregiver normally providing care. Respite can be provided in-home or out-of-home.

Resource center assists with the following services: - Advocacy: Includes areas of housing, civil and human rights, employment, transportation, and others. The Center advocates for state, local, and federal legislation. - Aging in Place Program: Offers accessible modification assistance, ramp installation, and accessibility assessments for homes and businesses for safe access and increased independence. - Assistive Technology Network: Assists individuals in the process of obtaining assistive technology devices. - Homemaking Services: Assists individuals in managing household activities including housekeeping, laundry, meal preparation, and shopping. - Independent living skills training - Information and Referral: Provides up-to-date disability information and referrals to various agencies and individuals within the region. - On-Demand Lending Library: Program will purchase the requested device and the individual can use it for as long as needed; the program retains ownership of the item. Eligibility requirements: first come, first served basis; funds are limited. - Peer Mentoring: Assists individuals living with a disability to identify goals and create a unique plan to achieve those goals. Home and Community Based Services Program - Advocate Homecare Community Service

- 24-hour emergency assistance: On-call counseling and problem-solving and/or immediate response for assistance at a person's home due to a health/personal emergency - Community residential services: Services that provide training and/or habilitation, ongoing residential care and supportive services to adults and/or children in a setting licensed by the lead agency. These services are individualized and based on the needs of the person, as identified in the support plan - Companion services: Non-medical care, supervision and socialization to a person age 18 or older - Crisis respite: Short-term care and intervention strategies provided to a person due to the need for caregiver relief, protection of the person or others living with the person, or the person's need for behavioral or medical intervention - Day support services: Individualized home, and community-based services (HCBS) that provide opportunities for community-based training and support for people with disabilities. Day support services develop and maintain essential and personally enriching life skills to ensure that people with disabilities can fully access and participate in personally preferred activities within their community - Day training and habilitation: Services that develop and maintain life skills for people with developmental disabilities or related conditions so they can fully participate in community life - Family residential services: Services that provide training and/or habilitation, ongoing residential care and supportive services to adults and/or children in a setting licensed by the lead agency where the license holder resides in the home. These services are individualized and based on the needs of the person, as identified in the support plan - Homemaker services: Eligible services range from light household cleaning to household cleaning with incidental assistance with home management and/or activities of daily living - Individual community living supports (ILCS): For people who need reminders, cues intermittent/moderate supervision or physical assistance to remain in their own homes - Individualized home supports without training: Services for adults or children when they need support, assistance and supervision in at least one of the community living service categories when living in their own home or family home. Community living service categories include community participation; health, safety and wellness; household management; adaptive skills - Night supervision: Provides overnight assistance and monitoring by staff in the person's own home - Respite care services: Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care.

Caregiver Support - Good Samaritan Society

Provides respite care to people who are caregivers needing time away from caregiving, whether on vacation or in need of rest. Respite care offers around-the-clock assistance during a short-term stay, typically lasting more than 24 hours and up to two weeks. It includes 24-hour care, balanced meals, social activities, and housekeeping, available at a community site or in the home of the person receiving care. Adult day services are available during the daytime to enable caregivers to work, run errands, or take a break from caregiving duties.

- Provides respite care when the caregiver desires it, provided that we have an available volunteer. They do not provide on-site respite care services. They do have some volunteers who will visit with people when their caregivers need a break, but they must be fairly self-sufficient. Faith in Action volunteers cannot provide any hands-on or personal care services.
