The Center advocates on state, local, and federal legislation Aging in Place Program - Offers accessible modification assistance, ramp installation, and accessibility assessments for homes and businesses for safe access and increased independence in people's homes and businesses Assistive Technology Network - Assist individuals in the process of obtaining assistive technology devices - On-Demand Lending Library: Program will purchase the requested device and the individual can use it for as long as needed, the program retains ownership of the item. Eligibility requirements: first come, first served basis; funds are limited Homemaking Services - Assist individuals in managing household activities including housekeeping, laundry, meal preparation, and shopping Information and Referral - Up-to-date disability information and referrals to various agencies and individuals within the region Independent Living Skills training Peer Mentoring - Assists individuals living with a disability to identify goals and work to create a unique plan to achieve those goals - Match interested consumers with a peer mentor who provides support, provide problem-solving solutions, social skills, etc. Public Education - Provide information, resources, and materials on accessibility guide, ADA, disability laws, etc. Respite Care Services - Provide short-term in-home care and support when the primary caregiver or family members need to be absent from the home. Systems Advocacy (to implement changes where needed) Transition/Relocation Services - Assist individuals living with a disability in transition from nursing facilities and other institutions into community settings with the goal of preventing institutionalization

CARE provides trained respite workers to stay with a loved one and provide a well deserved break for caregivers. Workers are available days, evenings, weekends or overnights
Provides resources for seniors living alone as well as caregivers needing support. Services are designed to help maintain independence as long as feasible. Home-based services provided according to availability of volunteers and staff: - Assisted Transportation: Short distance volunteer drivers provide help to access medical appointments, groceries, pharmacy, etc. Limitations and restrictions apply. - Companionship: Home visits provided by volunteers to provide non-medical support to encourage socialization - Homemaking: Indoor light housekeeping services include mild laundry services, floor care, kitchen/bathroom cleaning and other indoor help. Light "fix it" services (i.e. changing light bulbs, smoke detector batteries, etc.), meal preparation, menu planning, picking up groceries and medications, help with organizing bills, etc. Supportive programs: - Caregiver Consulting: Helps caregivers manage and live their lives while caring for another person, develop individualized plans and connect with resources in the community - Caregiver Support Groups: Professionally facilitated, confidential environment for caregivers to share concerns, feelings regarding their role as caregiver and to learn from each other - In-Home Respite: Home visits by volunteers who visit with care receiver to temporarily relieve the caregiver. (Services are not home health aide, medical or PCA). - Social Respite: Memory Cafe meets once a month providing conversation, tactile arts/crafts projects, exercise, music and games to temporarily relieve the caregiver while providing socialization for care receivers

Waivered Services: - Available to persons with a variety of disabilities - Waivered services are flexible and can be provided in a variety of settings - CADI (Community Alternatives for Persons with Disabilities) Waiver is available to adults and children with disabilities who need a nursing facility level of care: - TBIW (Traumatic Brain Injury Waiver) is available to adults and children who have had severe brain injuries from an accident, or due to stroke, heart attack or disease - MR/RC (Mental Retardation and Related Conditions) Waiver provides supports to adults and children with developmental disabilities - EW (Elderly Waiver) provides supports to seniors at home or in adult foster care who would otherwise have to live in a nursing home - AC (Alternative Care) Waiver provides supports to seniors at home or in adult foster care who would otherwise have to live in a nursing home, who because of their assets don't qualify for traditional Medical Assistance programs. Waivers pay for services such as personal care assistants, independent living skills counseling, homemaking, respite care, in-home family support, and other services Personal Care Attendants: - Provide assistance with daily living activities such as hygiene, grooming, dressing, taking medications, simple housekeeping, and other tasks - PCAs also provide behavioral monitoring and supervision to help clients with behavior problems - PCAs can help clients with FAS to live in a variety of settings, including foster care, their own homes and apartments, and family homes Other services: - In-home family supports and respite RSI provides support to caregivers and families of people with disabilities - Respite services gives caregivers and families a break for a few hours to a few days at a time. Respite can be provided in or outside of the family home - In-Home family supports services helps adults and children with daily life management skills, such as self-care, socialization, sensory/motor development, behavior management, community living and healthy recreation. - Respite and in-home family supports are waivered services programs - Adults and children who do not qualify for the MR/RC waiver may be eligible to receive similar kinds of help through the CADI or PCA programs

The Living At Home Block Nurse Program provides or arranges whatever support an elder neighbor needs to continue living at home as long as possible. This support, which may otherwise not be available, uses the program's small staff, volunteers and/or paid nursing staff. Services include: - Advocacy - Blood pressure checks - Caregiver and respite - Case management - Door-to-door transportation provided by volunteers. Allows the caregiver time to attend to other tasks and provides contact for the home-bound person Caregiver support group: Helps spouses and family members with changes in the family unit; once a month in both Ely and Babbitt Friendly visits: Volunteer visits for area residents who might be homebound or just alone and want or need contact with the greater community Grocery delivery: Available for those who find it difficult or unsafe to navigate local grocery stores. Volunteers will deliver groceries to the home Northwoods memory care program: Guides clients and their families through Alzheimer's by helping to reduce stress and increasing family support Tasty Tuesday: Provides a class to older adults at no cost at the Ely senior center. The class provides healthy recipes, food preparation, and more. Availability is limited. Volunteer Rides: Coordinates rides that are not available through other services for local and long distance medical appointments, meetings, errands, and social events.
