- 24-Hour Responder Service: On-call staff help with health and personal injuries. The service is often paired with Independent Home Support with Training program. Staff will respond by phone or in person depending on the level of services. - Acute Behavioral Services for Children and Adults: Services provided to individuals with interfering behaviors and/or anti-social issues, who have a history of law enforcement involvement, have difficulty living with others and require high staffing needs, are in frequent crisis situations, or coming out of an institution. - Case Management: Available to residents of Hennepin, Ramsey, and Wright Counties who receive funding through various waivers and/or programs. Assists navigating the social service system and provides information and advocacy. - Integrated Community Supports: Provides support and training/habilitation in community living service categories to adults ages 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person's living unit or in the community. - Individualized Home Supports: Provides a blend of in-person and remote services that focus on support and training of a person who needs to maintain or increase independence for their continued experience living in the community. Also offers services to relatives working to support the person's goals. - Residential Services: Operates residences for adults with significant behavioral, medical, or physical challenges including brain injuries. Residential group home programs are licensed as adult or child foster care, or a supervised living facility. Each site serves 2 - 6 people. - Supportive Employment Services: Provides individuals with intellectual and developmental disabilities assistance with initial stages of the employment assessment period, job search and development, job coaching, and working independently - Vocational Rehabilitation Services: Offers a variety of services including job-seeking skills training, job coaching, social coaching, post-secondary education, and more. Adult In Home Respite Care
Volunteers attend to the non-medical needs of people and their families faced with the end stages of serious illness. During their visits, volunteers may play cards, share a movie, converse or listen to music with the patient; prepare a light meal; help with housekeeping tasks; or read to the patient. This gives the opportunity to family caregivers to take a break from their caregiving responsibilities. Volunteer caregivers help ailing people stay at home longer by eliminating caregiver burnout. Based on the patient's specific needs, on average, a person can count on two - four hours per week of their volunteer's time.

The Center advocates for state, local, and federal legislation. - Aging in Place Program: Offers accessible modification assistance, ramp installation, and accessibility assessments for homes and businesses for safe access and increased independence. - Assistive Technology Network: Assists individuals in the process of obtaining assistive technology devices. - Homemaking Services: Assists individuals in managing household activities including housekeeping, laundry, meal preparation, and shopping. - Independent living skills training - Information and Referral: Provides up-to-date disability information and referrals to various agencies and individuals within the region. - On-Demand Lending Library: Program will purchase the requested device and the individual can use it for as long as needed; the program retains ownership of the item. Eligibility requirements: first come, first served basis; funds are limited. - Peer Mentoring: Assists individuals living with a disability to identify goals and create a unique plan to achieve those goals. Matches interested consumers with a peer mentor who provides support, problem-solving solutions, social skills, etc. - Public Education: Provides information, resources, and materials on accessibility guides, ADA, disability laws, etc. - Respite Care Services: Provides short-term in-home care and support when the primary caregiver or family members need to be absent from the home. - Systems Advocacy: Works to implement changes where needed at a broader systemic level. - Transition/Relocation Services: Assists individuals living with a disability in transitioning from nursing facilities and other institutions into community settings, with the goal of preventing institutionalization.

Waivered Services: - Available to individuals with a variety of disabilities - CADI (Community Access for Disability Inclusion) Waiver supports individuals with disabilities to live as independently as possible in their communities - Waivered services are flexible and can be tailored to meet individual needs in a variety of settings Waiver Programs for Individuals Who Need a Nursing Facility Level of Care: - AC (Alternative Care) Waiver supports older adults who do not qualify for traditional Medical Assistance due to assets but would otherwise require nursing home care. Services may include personal care assistance, independent living skills training, homemaking, respite, and in-home family support. - EW (Elderly Waiver) supports older adults who would otherwise need to live in a nursing facility, helping them remain in their homes or community-based settings - MR/RC (Developmental Disabilities) Waiver provides services to individuals of all ages with developmental disabilities or related conditions, supporting them in living meaningful and inclusive lives - TBI (Traumatic Brain Injury) Waiver supports individuals who have experienced a significant brain injury due to accident, stroke, illness, or other causes, helping them live as independently as possible in the setting of their choice Personal Care Assistants (PCAs): - PCAs assist individuals with daily living activities such as bathing, dressing, taking medications, light housekeeping, and meal preparation - PCAs may also provide support with behavioral needs, helping individuals manage challenges in positive and supportive ways - PCAs can support individuals in a variety of settings, including their own homes, family homes, or foster care placements Other Services: - Individuals who are not eligible for the Developmental Disabilities Waiver may still qualify for similar supports through the CADI or PCA programs - In-home family support services help individuals build daily life skills such as self-care, communication, social interaction, sensory and motor development, and community living skills - In-home family support and respite services provided by RSI (or other organizations) offer valuable assistance to caregivers and families supporting individuals with disabilities - Respite services offer caregivers a temporary break, from a few hours to several days, either in the family home or in other supportive settings - Respite and in-home family supports are often part of waivered service programs and can be personalized to fit the needs and preferences of the individual and their family

Addresses the conditions of emotional disturbance that impair and interfere with an individual's ability to function independently. For children with emotional disturbances, rehabilitation means a series or multidisciplinary combination of psychiatric and psychosocial interventions to: - Restore a child or adolescent to an age-appropriate developmental trajectory that had been disrupted by a psychiatric illness; or - Enable the child to self-monitor, compensate for, cope with, counteract, or replace psychosocial skills, deficits or maladaptive skills acquired for a psychiatric illness.

Provides homes and services to people with developmental and delayed disabilities, mental illness, traumatic brain injuries, and other health-related needs wherever identified in communities throughout Minnesota. Individuals can participate in day placement, day treatment, and vocational readiness on a regular basis. Adult foster care - Provides food, lodging, and supervision services to a person with disabilities, mental illness, and brain injury. These services can be short term or long term. 62 residential program sites/ 261 beds. 12 sites offer a 5th bed or apartment with services, for 25 total beds. 4 AFC beds. Children's foster care - A 24-hour substitute care for children placed away from their parents or guardians and for whom the state agency has placement and care responsibility. 4 residential program sites/8 beds. Community-based services - Community support programs - Independent living services - In-home services, including training for of the person and family members to increase their capabilities to care for and maintain the person in their home - Personal care assistant - Respite care services - Supported employment - Supported living services Intensive residential treatment - Short term for up to 90 days. 10 bed facilities at Brentwood (Marshall) and Fresh Start (Duluth). Brentwood has 1 of the 10 beds as a swing bed (crisis and IRT bed). Fresh Start serves an all-female population. 20 total IRT beds. Intellectual disabilities - Provides an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID). Individual must have a developmental disability or related condition by their 22nd birthday and requires a 24-hour plan of care. 10 facilities, 58 beds. 5 beds at Arlington in Fergus Falls and Rollingwood in North Mankato. 6 beds at Prairiewood, Rosewood, and Scenicwood in Alexandria; Lakewood in Spicer; Northwood in Red Lake Falls; Eastwood and Westwood in Redwood Falls; and Ravenwood in Morris.

Provides 245D waivered services which include: - Homemaker Services: Provides light household cleaning, laundry, assistance with home management, and activities of daily living - Individual Community Living Supports (ICLS): Offers assistance to older adults who need reminders, cues, intermittent/moderate supervision, or physical assistance to help them remain in their homes - Individual Home Supports Without Training: Provide support, assistance, and supervision to adults or children who live in their own homes - Night Supervision Services: Provides overnight assistance and monitoring in the client's home when the client is in need of assistance with activities of daily living, positive support programming, transition plans, and reinforcement of skills development - Personal Support: Non-medical care, assistance, and supervision
