Homeless Services - Range Mental Health Center

Provides homeless outreach and case management services to adults with serious and persistent mental illness. Offers housing options including crisis beds, rental assistance, and permanent housing. Outreach, case management, and rental assistance are also available to homeless youth under the age of 24. NOTE: Single adults and youth must get a pre-screen through United Way 211 to access this program. The pre-screen information is provided to Saint Louis County Housing Assessment Team (SLCHAT).

Sexual Violence, Domestic Abuse, and Trafficking Advocacy Services - Minnesota Indian Women's Resource Center

Domestic Violence Prevention and Intervention: An intervention and healing program for Native women, girls, and Two Spirit people who have experienced intimate partner violence, including domestic violence, dating violence, sexual assault, and/or commercial sexual exploitation. Offers counseling and community-based education activities to clients that encourage healing, build resilience, reduce risk of repeat traumas, etc. Healing Journey: A structured peer support group for adult American Indian women ages 18 and over who are challenged by chronic mental health, substance use, and trauma histories. ?Safe Harbor Initiative: A supportive program for youth ages 24 and under who have experienced sexual exploitation or are at risk of sexual exploitation. ?Sexual Assault Advocacy: Provides crisis intervention, individual and group counseling, forensic examination support, and court advocacy for American Indian survivors of sexual violence. Street Outreach: A collaborative street outreach program designed to facilitate a path towards a healthier life for people who are ready to make a positive change. Wawokiye: Provides outreach and services for youth and adults who are experiencing sex trafficking, unsheltered relatives, and those who need services realted to opioid addictions.

Crisis Response Team - Dodge and Olmsted Community Corrections

A collaboration effort within Olmsted County's Health, Housing, and Human Services division, in partnership with the Rochester Police Department and Olmsted County Sheriff's Office. Provides services that address public health concerns including mental health, poverty, and substance use. Community outreach specialist may provide residents with resources to address their needs, making the appropriate referrals, and follow up with them and their providers to ensure their needs were met and properly addressed. Street Outreach - Duluth Street Outreach Team

Outreach workers are on call to meet with and talk to people who are homeless and staying outside or in places not fit for human habitation. Outreach workers provide supplies such as winter clothing and camping gear (when possible) to meet people's basic needs, and try to connect people to appropriate services to improve their quality of life. Accepts donations of winter clothing and camping gear. Outreach workers can provide information on how to schedule a housing assessment for Coordinated Entry transitional or permanent supportive housing.

Resource Centers: During designated drop-in hours, YMCA Resource Centers are staffed by an Outreach Worker who can help youth, ages 12 - 24, with basic needs support such as food, clothes, hygiene items, household supplies, parenting items, and more. Also during drop-in hours, youth can stop in to get their coordinated entry assessment completed, help find shelter, be screened for prevention services, or access other YMCA resources/services. Four locations: Lion's Resource Center/Northeast Minneapolis, Elliott Park, Emma's Place in Coon Rapids, and North Community YMCA. Safe Stay Emergency Shelter: Moves youth off the streets into a safe space and provides supportive services. School and Street Outreach: Youth Outreach Workers provide services for youth and young adults who are homeless or at risk of homelessness at schools and out in the community to meet immediate needs and connect them to community resources. Street Outreach Program - Agate Housing and Services

Works with individuals sleeping outside or in substandard places and assists them to get into permanent housing. Provides a non-emergency response to homeless concerns in public settings with a mobile outreach team able to visit individuals in the community. Can help address immediate basic needs for safety and survival outside with items such as food, personal grooming supplies, outdoor gear, clothing, and shelter referrals. Low income and individuals experiencing homelessness can pick up a free Handbook of the Streets from 9 am - 4 pm Monday - Friday or download the PDF from the website.

Street Outreach and Education - Breaking Free

Provides street-based outreach to individuals in prostitution to help build rapport, and provide hygiene kits, condoms, and other seasonal items in an effort to meet individuals' ongoing needs. Trains stakeholders, including law enforcement, judges, corrections, schools, social workers, the medical community, and the hospitality industry, among others. Also, canvas areas in Minneapolis and Saint Paul that are known to be high traffic areas for prostitution.

For chronically homeless individuals living on the street, the transition from the street to permanent housing is made in stages (as necessary) and staff work to create a service environment where individuals feel at ease, out of danger, with flexible program services to meet their needs. Create a one touch point for clients to receive multiple services through enhancing service integration to People Incorporated services or community resources. Behavioral Health Unit - Duluth Police Department

Mental Health Unit (CORE): Responds to mental health calls. An officer and a homeless advocate team up to work together to get individuals to the right resources. Substance Use Response Team (SURT): Provides post-overdose outreach. Program divert individuals with substance use disorders away from the criminal justice system and into services that are supportive of their recovery. This is a peer recovery-led and run outreach program that does not include police in follow-up attempts but is instead embedded within a law enforcement agency.
