Active Seniors: Provides adults 55+ with a variety of diverse activities such as exercise, wellness, and social/recreation designed to support independence Care Consultation: Keystone Care Consultants help seniors navigate their health, economic, and social needs. Care managers meet with individuals in their homes or at one of the community centers to conduct an assessment, gather input from caregivers, and create goals. Once the assessment is complete, services may include: setting up rides to a social program, researching housing options, making a referral to an agency that helps with grocery shopping and/or cleaning, providing caregiver support, and accompanying seniors to doctor visits. Grocery Delivery: Provides free grocery delivery for low-income seniors and adults with disabilities. Those who qualify can receive monthly deliveries. Delivery can include meat, dairy, fresh produce, and shelf-stable items. In each delivery, individuals will receive an average of 25 pounds of food per person in the household. Accommodations are based on customer choice models such as vegetarian, halal, etc. depending on what is available. Deliveries will be dropped off between 12:15 pm and 1:30 pm Tuesday - Thursday by a volunteer.
