Once the assessment is complete, services may include: setting up rides to a social program, researching housing options, making a referral to an agency that helps with grocery shopping and/or cleaning, providing caregiver support, and accompanying seniors to doctor visits.
In-Home Help: Social workers coordinate with Recover Health to provide seniors with in-home nursing care, in-home therapies, and home health aides to assist with daily care.
