FHPAP does not provide direct assistance. Hennepin County is responsible for determining eligibility and distributing rent assistance funds. - On-site health clinic and health care support including referrals to mental health and substance abuse services - Street Outreach Program: Caseworkers go out into the community, locate homeless youth and inform them of services that are available to them. May provide the youth with clothing, safe sex information and resources, bus cards, etc.
