Residential services including IRTS and crisis services including staffing, client advocacy, independent living skills training, home-like settings, medication supervision, and education. - Maureen's House: crisis residence, supports individuals in psychiatric or other crises stabilize in the community without becoming homeless, and avoid hospitalization where possible. Length of stay ranges from 1 - 10 days. Clients are accepted from the greater metro area for rooms when available. - Guild South: Intensive Residential Treatment Services (IRTS) supports individuals to develop and enhance psychiatric stability, personal and emotional adjustment, self-sufficiency, and skills needed to live in a more independent setting. The length of stay can be up to 90 days.

A 12-bed Intensive Residential Treatment (IRT) program for adults in need of behavioral health and support services. The focus of this program is to help people with a mental or chemical illness avoid hospitalizations or promote continued stabilization following a hospitalization. Staff utilizes evidence-based practices - illness management and recovery, integrated dual disorder treatment, therapy, psychiatry, counseling, etc. - to help residents create their treatment plan and chart a path for recovery. The residential program provides a variety of classes to assist residents in their recovery efforts, including: - Family psycho-education skills - Coping skills - Problem solving - Daily living skills - Healthy choices for managing life stresses

Nine-bed adult residential crisis stabilization unit provides a short-term safe place for individuals to stay while working through a mental health crisis. The unit is staffed 24 hours/7 days a week and offers counseling and education to clients in managing their mental health symptoms. All admissions are voluntary and based on bed availability.

Residential Crisis Stabilization hybrid services integrate mental health, medical, and substance use care in a 24-hour, supervised setting. Provides short-term crisis stabilization services in the same setting. Diane Ahrens provides a home-like environmental in keeping up with a recovery orientation. Short-term counseling is also provided related to the immediate crisis, psychiatric evaluation, medication evaluation and rehabilitation services (i.e. skill building to restore a person to a pre-crisis level of functioning). Services are provided one-to-one as well as in groups scheduled throughout the day. Staff also assist individuals with connecting to other community services to build the individual's support network. NOTE: Beds are updated daily via the website. Adult residential crisis stabilization (RCS) provides structure and support to an adult client in a community living environment when a client has experienced a mental health crisis and needs short-term services to ensure that the client can safely return to the client's home or pre-crisis living environment with additional services and supports identified in the client's crisis assessment. Residential crisis stabilization is provided in a 24 hour licensed residential setting by qualified mental health staff. Residential crisis stabilization serves eligible members assessed during a crisis assessment to be experiencing a mental health crisis.
Adults and youth ages 6 - 17 who are experiencing a mental health crisis. Provides a supportive environment for individuals needing a safe place to recover and stabilize during a crisis. Individuals staying at Crisis Stabilization will meet with a qualified mental health professional, mental health practitioners, and nursing staff during their stay. Services offered include skills training in individual and group settings, evaluation, stabilization, and referrals. Average length of stay is 1-10 days.

Community and regional collaboration which provides residential crisis stabilization for adults experiencing a mental health crisis or emergency. This includes programming designed to enhance psychiatric stability and emotional adjustment, and the skills to return to a more independent setting. Residential Crisis Stabilization - 24 hour nursing care - Crisis assessment, support, and intervention - Discharge/aftercare planning with referrals to further aftercare treatment - Family support - Medication administration, management, and monitoring - Psychiatric medical assessment - Recovery focused care - Short-term problem solving and rehabilitative services - Symptom management and relapse prevention Individuals admitted work with a mental health professional, or practitioner on a daily basis. A residential crisis stabilization facility that provides up to a 10-day voluntary stay. Guests have access to a range of therapeutic services and activities to promote wellness and recovery from individual counseling sessions to group therapy, mindfulness practices, and recreational activities.
Provides homes and services to people with developmental and delayed disabilities, mental illness, traumatic brain injuries, and other health-related needs wherever identified in communities throughout Minnesota. Individuals can participate in day placement, day treatment, and vocational readiness on a regular basis. Adult foster care - Provides food, lodging, and supervision services to a person with disabilities, mental illness, and brain injury. These services can be short term or long term. 62 residential program sites/ 261 beds. 12 sites offer a 5th bed or apartment with services, for 25 total beds. 4 AFC beds. Children's foster care - A 24-hour substitute care for children placed away from their parents or guardians and for whom the state agency has placement and care responsibility. 4 residential program sites/8 beds. Community-based services - Community support programs - Independent living services - In-home services, including training for of the person and family members to increase their capabilities to care for and maintain the person in their home - Personal care assistant - Respite care services - Supported employment - Supported living services Intensive residential treatment - Short term for up to 90 days. 10 bed facilities at Brentwood (Marshall) and Fresh Start (Duluth). Brentwood has 1 of the 10 beds as a swing bed (crisis and IRT bed). Fresh Start serves an all-female population. 20 total IRT beds. Intellectual disabilities - Provides an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID). Individual must have a developmental disability or related condition by their 22nd birthday and requires a 24-hour plan of care. 10 facilities, 58 beds. 5 beds at Arlington in Fergus Falls and Rollingwood in North Mankato. 6 beds at Prairiewood, Rosewood, and Scenicwood in Alexandria; Lakewood in Spicer; Northwood in Red Lake Falls; Eastwood and Westwood in Redwood Falls; and Ravenwood in Morris.

Services include short-term or long-term residential treatment alternative to hospitalization, or rental assistance and supportive services for people living in the community. Housing options facilitated by RADIAS Health include: Intensive Residential Treatment Services: Long-term residential housing, 30 - 90 day stay - Carlson Drake House: Program located in Bloomington. Staff provide recovery-oriented, evidence-based treatment and rehabilitative services for twelve adults with a primary diagnosis of mental illness. Program services offer an alternative to hospitalization. - Community Foundations: Program located in Maplewood. Staff provide recovery-oriented, evidence-based treatment and rehabilitative services for sixteen people struggling with acute symptoms of mental illness and/or co-occurring substance use disorders. - ReEntry House: Program located in south Minneapolis. Staff provide recovery-oriented, evidence-based treatment and rehabilitative services to adults with a primary diagnosis of mental illness. Crisis Stabilization: Short-term stay of 3 - 10 days for residential crisis stabilization - ReEntry Crisis Housing: Program located in south Minneapolis. Program services to adults who are experiencing a mental health crisis. Team consists of mental health professionals, practitioners, nurses, and a peer recovery specialist to meet individual needs.

The Steiner Kelting Mental Wellness is a 12-bed residence providing Residential Crisis Stabilization Services (RCS) and Intensive Residential Treatment Services (IRTS). RCS services help individuals manage immediate mental health needs, develop a crisis prevention plan, learn new coping skills, and recognize symptoms earlier. IRTS services are designed to help enhance psychiatric stability, personal and emotional adjustment, self-sufficiency, and independent living skills for individuals who often have co-occurring mental health and substance use diagnoses. A person-centered treatment plan is developed to help individuals set goals to achieve independence and lifelong wellness. Services include: - 24-hour on-site staff and assistance in a residential setting - Assessment of immediate needs and factors that lead to the crisis - Individualized treatment to address immediate needs and restore the individual to pre-crisis level of functioning - Individual and group therapy - Living skills/foundational wellness development including exercise and nutrition - Medication management - Crisis assistance, development of health care directives, crisis prevention plans, and relapse prevention plans - Inter- and intra-agency case coordination - Transition and discharge planning.
