Crisis Services - Alluma

Provides crisis services and a crisis helpline for individuals experiencing a mental health crisis. Services include: - 24 hour/7 days a week support and crisis helpline to screen for safety, stabilize a crisis, support and provide appropriate referrals. All calls are free, confidential, and anonymous. - Mobile crisis response team to provide face-to-face crisis services, including assessment, intervention, and stabilization. The mobile crisis response team works in conjunction with local law enforcement agencies and hospital emergency departments. Alluma partners with local law enforcement agencies to respond to 911 calls involving individuals experiencing a mental health crisis. - Supervision and observation through the Crisis Stabilization Unit, a two-bed unit available for adults who need a safe place to recover during a mental health crisis. Provides access to services such as case management, counseling, skills building, and coordination of services to local community resources. Only serves adults ages 18 years and older who volunteer to be admitted to the Crisis Stabilization Unit, are medically stable, and non-aggressive. Does not treat acute medical conditions.

Three adult mental health crisis response services are defined as: - Crisis Assessment, - Mobile Crisis Intervention, and - Crisis Stabilization services This service provides adult mental health mobile crisis response services to eligible recipients. A mental health crisis is a behavioral, emotional, or psychiatric situation that would likely result in significantly reduced levels of functioning in primary activities of daily living; an emergency situation; or placement of the recipient in a more restrictive setting. This setting could include but not be limited to, inpatient hospitalization. A mental health emergency is a behavioral, emotional, or psychiatric situation, which causes an immediate need for mental health services.

Emergency assistance includes, but is not limited to, the following expenses: - Cleanup of a crime scene - Purchase and installation of necessary home security devices - Reimbursement for reasonable travel and living expenses that the victim incurred to attend court proceedings that were held at a location other than the place where the crime occurred due to the change in venue - Reimbursement of towing and storage fees incurred due to impoundment of a recovered stolen vehicle - Replacement of necessary property that was lost, damaged, or stolen as a result of a crime - Transportation to locations related to the victim's needs as a victim, sch as medical facilities and facilities of the criminal justice system. Victims of crime may receive funding through the program for items such as repairing a broken door, installing new locks, replacing clothing, or bedding taken as evidence.

Family Response Services - Hennepin County Human Services - Social Services

Hennepin County works with community agencies to deliver the Family Response service. Family Response can help, with immediate, in-person support and stabilization for children, youth, and their parents/caregivers. The model's goal is to reduce hospitalizations and other out-of-home placements by providing families with immediate, upstream support. There are no specific criteria required to initiate services, but concerns may include children and youth who are: - Experiencing worry and concern - Feeling down or less engaged - Having a hard time with a family member or change in the home - Having issues at school - Showing aggression or anger When a call is made to Family Response, staff will gather basic information and send a family response team to the family's location within one hour. The Family Response team will: - De-escalate and address the immediate concern - Engage and support the family over the next 72 hours - Connect families to support, including an optional 8-week stabilization service. Stabilization services can provide healthy, safe behaviors, and help the child manage their daily activities.

Crisis response offered through a partnership between Zumbro Valley Health Center, South Central Human Relations Center, and Hiawatha Valley Mental Health Center. Counselors help individuals reach a place of emotional and physical safety, and then direct the them to community resources that offer longer-term support. They also provide over the phone assessments, safety plans and support. Counselors connect individuals to local teams able to meet the individual where they are and provide additional support. Mobile teams complete confidential comprehensive crisis assessments. A crisis assessment is an immediate, face-to-face evaluation by 2 crisis-trained mental health practitioners consulting with a mental health professional. The assessment helps do the following: - Assess current risks and life events - Assess level of functioning - Assess mental health symptoms - Assess the protective factors that the individual has available in the crisis - Assess mental health symptoms - Provide a support network - Take into account cultural considerations Provides immediate intervention to aid the individual in relieving current symptoms of the mental health crisis.
