Case managers work closely with clients to foster long-term well-being and independence by identifying specific needs, connecting them to valuable community resources, and supporting them reach their personal and wellness goals. - Children's Case Management Services (CTCM): A voluntary service aimed at providing holistic support for children facing emotional challenges. Assertive Community Treatment (ACT): ACT is an evidence-based treatment for adults who have a severe and persistent mental health illness. Based on severity of symptoms and impairments, clients work with the team on a long term basis. Adult Rehabilitation Mental Health Services (ARMHS): Adult mental health services that enable individuals to develop and enhance psychiatric stability, social competencies, personal and emotional adjustment, and independent living and community skills. Behavior Health Home: Provides individuals with their care needs. The team work closely with the individual, their supporters and providers to ensure comprehensive quality of care. Community Support Program (CSP): The Community Support Program is a coordinated care and treatment program which provides a wide range of individualized treatment, rehabilitation and support services to persons with serious and persistent mental illness. The purpose of the community support program is to assist individuals to live as successfully and independently as possible in their own communities by supporting independent living and coping skills. Targeted Case Management: Helps adults with serious and persistent mental illness (SPMI) gain access to needed mental, medical, social, educational, vocational and other necessary services as it relates to the person's overall health needs.

- Rehabilitative mental health services that enable the recipient to develop and enhance psychiatric stability, social competencies, personal and emotional adjustment, and independent living and community skills, when these abilities are impaired by the symptoms of mental illness. - ARMHS are also appropriate when provided to enable a recipient to retain stability and functioning, if the recipient would be at risk of significant functional decompensation or more restrictive service settings without these services. - Adult rehabilitative mental health services instruct, assist, and support the recipient in areas such as: interpersonal communication skills, community resource utilization and integration skills, crisis assistance, relapse prevention skills, health care directives, budgeting and shopping skills, healthy lifestyle skills and practices, cooking and nutrition skills, transportation skills, medication education and monitoring, mental illness symptom management skills, household management skills, employment-related skills, and transition to community living services. Basic living and social skills is a separately billable service as defined below. - These services may be provided to a recipient on a one-to-one basis in the recipient's home or another community setting or in groups.

The goal is to meet the mental health needs of White Earth Tribal members and their families by increasing family interactions, healthier lifestyles, and positive communication for all members. Services include: - Adult Rehabilitative Mental Health Services (ARMHS): Holistic approach that places the person at the center of all interventions. The goal is to restore each person's ability for independent living, socialization and effective life management. ARMHS instructs, assists, and supports the person in areas such as medication, education, and monitoring basic social and living skills. Assistance is also provided with mental illness symptom management, household management, employment support and transition to community living. - Assessments and evaluations for mental health. Diagnostic assessments and psychological testing for all ages. - Case management services - Children's Therapeutic Services and Supports (CTSS) to provide psychotherapy and skills training - Crisis intervention - Mental health counseling for adults, children, and families. Services are based on individual, child, and family needs.

Community Care Services Team: Specializes in supporting individuals with mental health and substance use disorders who are transitioning back into the community after extended or repeated stays in hospitals or other treatment facilities. Coordinates care across providers and systems and helps with assessment, service planning, treatment plan development, implementation, and ongoing evaluation. The team collaborates with county agencies, hospitals, mental health providers, primary care, and community services to promote stability and prevent rehospitalization.
