Provides several treatment programs for adults who have a problem with substance use. Services include:- Co-occurring program provided to adults with both substance use disorder and mental health diagnoses at the Oakdale location- Group/individual outpatient treatment such as New Choices for Recovery Program. The program helps adults take action to change their thinking and behaviors to both prevent relapse and form a more positive relationship with themselves, others, and their community. Approaches include cognitive therapy, motivational interviewing, and 12-step support. Also addresses co-occurring mental health issues.- Sober housing with permanent and supportive housing services to individuals who are homeless and single parents and their children in Forest Lake. The goal is to help parents and their children become self-sufficient and contributing members of the community.- Substance use assessments

Project CHILD is for pregnant people ages 16 and older or who have been reported to Child Protection for controlled substance use anytime during their pregnancy. The program can also support those already in active recovery to reduce the risk of relapse during pregnancy. Services offered:- Help with basic needs including transportation- Substance use assessments, treatment services, case management, and counseling for up to six months after delivery- Peer recovery support specialist services - people with a lived experience in substance use and recovery- Prenatal care referrals, health assessments, and individualized care planning with a nurse- Prenatal and postnatal support groups. Parent(s) learn about parenting and baby development while building community with other parents. Also, offer support for all caregivers.- Project CHILD collaborates with many county and community-based resources for services.NOTE: Pregnant women under the age of 16 should be reported to the Child Protection Screening Intake line.

Provides outpatient addiction treatment for adults with cognitive impairments, including drug and alcohol counseling using evidence-based approaches (individual and group therapy, expressive therapies, and mindfulness). Substance Use Disorder (Rule 25) and Problem Gambling (Rule 82) assessments are available by appointment. Problem gambling treatment is offered in person and via telehealth.

Provides 1 - 2 hour confidential assessments as a first step in determining if further chemical health services are needed. Recommendations, such as attending an educational class or treatment program, are offered in-house or through referrals to community providers.

Direct Access allows an individual to go directly to a provider they choose to receive a comprehensive assessment and access care immediately. The county will assess individuals who need help paying for a chemical use assessment or treatment services through the Behavioral Health Fund. Services use evidence-informed approaches to support recovery goals and individualized treatment planning.- Substance Use Disorder Assessments and Chemical Health Assessments: Provides assessments that are comprehensive evaluations used to determine appropriate levels of care and treatment recommendations. Assessments are provided for personal treatment planning as well as for legal, custody, and court related requirements. All assessments meet Minnesota standards and align with national guidelines.

Offers the Rule 25 on a walk-in and by appointment basis. The assessment lasts about an hour and includes questions about many different areas of a person's life. Role of the Rule 25:- First step in entering treatment- To assess a person's drug and/or alcohol use to determine if treatment is needed- To determine what level of care is most appropriate- To match a program that best meets the client's needs- Complete any insurance funding requirements. Whenever possible, the Assessor also speaks to a person that can also give information about the client being assessed.- Assessments meet court requirements

Chemical health assessments are conducted to determine whether individuals meet eligibility criteria for Avivo's outpatient treatment and recovery program. Services provided:- Family outpatient treatment with a housing option for pregnant individuals, parenting women, and fathers with dependent children ages 0 - 17. Provides access to counseling, case management, mental health services, substance use treatment, support groups, and other supportive services.- Individual, gender-specific, and culturally specific support for outpatient treatment with a housing option- Intensive outpatient 245G co-occurring treatment program for individuals who have a substance use disorder and a mental health diagnosis- Once enrolled in treatment and recovery services, individuals are eligible for enrollment in career education programs, employment services, and housing support.
