Direct Access allows an individual to go directly to a provider they choose to receive a comprehensive assessment and access care immediately. The county will assess individuals who need help paying for a chemical use assessment or treatment services through the Behavioral Health Fund. Services also include:

• Designated staff that provide chemical dependency assessments

• Referral to local community service providers for chemical assessments or treatment services. Also, directs individuals to visit www.fasttrackermn.org for additional resources.

The goal is to help clients reduce alcohol/drug use and achieve and maintain improved levels of social, economic, and personal development and independence. Direct Access allows an individual to go directly to a provider they choose to receive a comprehensive assessment and access care immediately. The county will assess individuals who need help paying for a chemical use assessment or treatment services through the Behavioral Health Fund. Services also include:

• Chemical dependency assessment

• Prepetition screening for all individuals petitioned for civil commitments for a chemical dependency due to being a danger to themselves or others

• Referrals to community-based service providers for chemical assessments or treatment services. Also, directs individuals to visit www.fasttrackermn.org for additional resources.

Provides Direct Access assessments, counseling, and support services to help individuals overcome substance use disorders. Direct Access allows an individual to go directly to a provider they choose to receive a comprehensive assessment and access care immediately.

Offers a wide range of mental health and substance abuse services, such as:

• Adolescent Co-Occurring Disorders Outpatient Program: Provides care for adolescents with a dual diagnosis of mental health and substance use disorders.

• Adult Day Treatment: An intensive day treatment program for individuals who are newly diagnosed or experience an increase in symptoms from prolonged illness. Groups meet three days per week for 3 hours each day.

• Lodging Plus Program: An intensive outpatient chemical dependency program located within the University of Minnesota Medical Center, serving deaf, deafblind, and hard-of-hearing clients embedded within the larger hearing population.

• Mental Health Emergency Services: Assists in behavioral emergencies, assesses, stabilizes, and offers next steps recommendations

• Mental Health Inpatient Treatment: Provides 24 hours/7 days a week care, acute crisis assessment, focused intervention, and discharge planning

• Mental Health Outpatient Treatment: Provides comprehensive mental health services for individuals struggling with a variety of mental health symptoms.

??- Mental Illness and Substance Abuse: An abstinence-based day treatment program. Groups meet Monday through Friday for three hours each day for about 6-8 weeks. Services include regular follow-up with a physician who specializes in mental health and addiction medicine.

• Partial Hospitalization: An intensive short-term program for individuals with anxiety and/or depression. Groups meet five hours a day, five days a week, for about 2-3 weeks. Individuals will have regular follow up with a psychiatrist.

• Senior Mental Health Inpatient Treatment: Offers a treatment program for older adults

• 55+ Day Treatment: Provides treatment to people experiencing mental health symptoms for the first time or those with a longer history of mental illness. Groups meet twice each week for four hours each day. This is not a dementia or recreational program.

Provides a full spectrum of outpatient services for adolescents and adults with substance use disorders or co-occurring substance use and mental health disorders. Services include:

• Case management

• Comprehensive assessments (including court-ordered/DWI/DUI assessments)

• Co-occurring disorder treatment programs

• Individual and family counseling

• Mental health counseling integrated with substance use treatment

• Outpatient group therapy

• Psychiatric evaluation and medication management

• Referral to inpatient or residential treatment when appropriate

Note: An initial assessment will be scheduled with a chemical health counselor to determine the most appropriate treatment plan.

The Human Development Center is providing the following services:

• Assessments for access to Residential Crisis Stabilization centers

• Comprehensive mental health and SUD assessments and access to providers

• Crisis screening, assessment, and stabilization

• Expedited access to primary care

• Intensive short-term case management and treatment coordination with support for follow-through

• Link to First Episode Psychosis program

• Mental health crisis line with a mental health crisis response team

• Peer support and peer recovery

• Psychiatric services and referral for ongoing psychiatric needs

• Streamlined assessment and access to medication-assisted treatment

• Support to access financial assistance, medical insurance, food support, emergency assistance, housing, employment services, child and family support services

Note: The mobile crisis response team only serves residents living in Carlton, Cook, Lake, and Saint Louis counties.

Assessment and outpatient chemical dependency treatment for adults, including:

• Alcohol

• Illegal and prescription drugs

• Intensive outpatient group therapy for substance use disorders, including individual, family, and group therapy; mixed gender, women, and men groups

• Co-occurring addiction and mental health problems

• Cognitive Behavioral Therapy

Assessment and outpatient treatment for adults with a compulsive gambling disorder, including:

• Individual and family therapy and interventions

• Cognitive Behavioral Therapy

• Group therapy including those affected family/concerned persons in the gambler's life

Teletherapy services available for chemical health assessments, comprehensive assessments, state-funded gambling services, participation in IOP, relapse prevention, and continuing care programming, individual mental health therapy, and diagnostic assessments

Provides substance use services delivered by licensed professionals. Services focus on assessment, treatment planning, and ongoing therapeutic support for individuals experiencing substance-related concerns. Provides the following substance use services:

• Chemical Health Counseling: Provides ongoing therapeutic support for individuals experiencing concerns related to alcohol or drug use. Services use evidence-informed approaches to support recovery goals and individualized treatment planning.

• Substance Use Disorder Assessments and Chemical Health Assessments: Provides assessments that are comprehensive evaluations used to determine appropriate levels of care and treatment recommendations. Assessments are provided for personal treatment planning as well as for legal, custody, and court related requirements. All assessments meet Minnesota standards and align with national guidelines.

Provides primary care and specialty medical services to patients. Services include:

• Allergy and Immunology

• Behavioral Health

• Cancer Screenings

• Colorectal Screenings

• Direct Access Testing

• Family Medicine

• Gynecologic Care

• Immunizations

• Laboratories

• Mammography

• Orthopedic Trauma

• Pediatrics

• Services include telephone counseling, mobile (in-person)crisis intervention, follow up counseling and referral to resources, and crisis stabilization

• Requests for intervention can be made by family members and community agencies

• Assists with problem resolution and coordination of increasing mental health services for individuals for whom this would be helpful

• Trained persons are available to the police and emergency wards to provide assessments of mental health emergencies, to develop an intervention plan, and arrange for any needed services identified in the plan

Following an intake process, clients receive medical services, addiction counseling and relapse prevention education. Clients are monitored for withdrawal symptoms and an assessment is completed, followed by recommendations and referrals to community programs.

• DWI Classes: Provides 8 hours of education on the effects of drinking and driving, along with tactics for improved decision making. Participants in this monthly program must be involved with traffic court or the Minnesota Department of Public Safety.

• Substance Use Counseling: Provides a series of comprehensive 1:1 and group treatment options

• Substance Use Outpatient Treatment Programs: Designed to help individuals overcome their dependence on alcohol and/or other chemicals. Options include 1:1 sessions with a licensed counselor, group therapy and ongoing support.

• Walk-in Assessment: Offers chemical dependency assessments

Provides intensive outpatient program with options for sober housing for men. Treatment plans include:

• 24/7 on-site staff

• Client-centered treatment in a homelike setting

• Continued care

• Group and individual counseling

• Relapse prevention program

• Substance use assessments/comprehensive assessments

• Transportation: Provided from any Minnesota location and many surrounding locations. Transportation is provided to and from Minneapolis/Saint Paul (MSP) airport for admission purposes. Transportation is also available to and from work for Elk River housing clients.

• Cognitive Behavioral Therapy Groups/Individuals: Provides CBT classes and individual education

• DWI Clinics: Assists individuals in meeting DWI class requirements through videos and lectures

• Public Speaking: Delivers education and awareness presentations tailored to the audience

• Workplace Trainings: Conducts trainings on the effects of alcohol and drug use in the workplace.

Medicated assisted treatment and withdrawal management provided to conduct diagnostic assessments and treatment coordination. Licensed to prescribe Suboxone and Naloxone.

Provides support to families struggling with substance use to help ensure healthy pregnancies. Intensive in-home services to pregnant individuals who are abusing chemicals including:

• Chemical dependency assessments

• Referrals to treatment and placement

• Counseling and referral to community resources

• Prenatal and aftercare services

• Support groups weekly, transportation and child care provided
