Rule 25 Chemical Health Assessments - Ross Resources, LTD

A Rule 25 Chemical Health Assessment may be ordered by a court, or an individual may want to pursue an assessment on their own to determine if there is an issue with their chemical use. If a person is deemed by an assessor to be in need of treatment, at any level, a referral and placement in a treatment center of the client's choice may occur. Ross Resources is one of the Itasca County contracted agencies to provide these assessments to both youth and adults who are Itasca County residents. A referral/intake form will need to be completed and insurance information will be gathered. If an individual does not have insurance, they are still eligible for treatment dollars through the Consolidated Fund should they qualify.

Provides outpatient chemical dependency treatment services to youth struggling with chemical dependency and mental health disorders. Offers services that are evidence-based practice models by addressing both mental health and substance use through collaborative treatment plans. Prevention 4 Teens is an education-based substance use prevention program for youth who are experiencing problems associated with alcohol or other drug use but do not meet diagnostic criteria for substance use disorder or require more formal interventions. Offers guidance, education, and support for families through two sessions with a counselor.

Health Care for the Homeless/House Calls - Minnesota Community Care

Increases access to medical services by traveling from site to site to bring services where it is convenient for individuals facing homelessness. Services include: - Chemical health assessment and referrals - Chiropractic services - Medical services include primary care, transportation assistance, and community resource assistance and referral - Nutrition services include assessment and referral, education, consultation, advocacy - Mental health diagnosis and treatment services - Optical including vision screening and eyeglasses - Pharmaceutical services - Physical therapy - Public health nursing services include outreach and assessment, maternal/child health and family planning, child and teen checkups, and TB case management Donations of unopened medicine, inhalers, and supplies are gratefully accepted. Must be delivered to the project.

Telewellness - Bridges Health Winona

Bridges Health is a student-led, faculty-guided, interprofessional clinic that provides a variety of free preventive health and wellness services. Services include: blood pressure screening, depression screening and emotional well-being assessments, diabetes screening, general assessment for substance use disorders, general physical exams, healthy exercise assessment, healthy sleep assessment, information and assistance, nutrition assessments, respiratory/asthma screening, school and sports physicals, skin cancer screening, support and socialization, and vision and hearing screening. Caring Calls provides weekly phone or videochats to those looking to discuss health and social needs or for socialization. Mental health support available in groups and individual appointments to support needs in emotional wellbeing.

Assessment Unit - Leech Lake Band of Ojibwe

- Provides Rule 25 Chemical Health Assessments for Native American people living within the boundaries of Leech Lake Reservation. - Provides assessments for ages 11 through adult. - Referrals are received through self-referrals; courts; probation; Child Welfare; and sometimes through detox centers, hospitals and schools. - Works with approximately 45-50 providers statewide, as well as referrals to the various Leech Lake CD programs. - Clients are referred to Inpatient Treatment, Outpatient Treatment, Aftercare and Relapse Prevention programs, and Halfway Houses. - Collaborates with treatment centers, social workers, probation officers, hospitals and schools to make the best possible plans for clients to obtain and maintain sobriety. - Priority assessments for pregnant women and adolescents. - Civil commitments for clients who are very ill due to chemical/alcohol use. - Data privacy and confidentiality are strictly adhered to according to policy.
