Services include the following: - Assistance obtaining medical equipment and supplies - Assistance with Medicare, Medicaid or commercial insurance billing - Coordination of community resources - Home health aides provide personalized support for daily life activities, such as bathing and dressing. - Home infusion therapy for patients requiring intravenous medications - Home safety evaluation and fall prevention education - Home telemonitoring - Lab tests - Medical social services - Nursing services (including assessment, patient education, medication management and skilled nursing procedures) - Nutrition counseling - On-call nursing consultation 24 hours/day - Physical, occupational and speech therapy - Physician communications - Wound care and educational support for ostomy and wound management Note: Does not provide Personal Care Assistance (PCA) services.
