North Homes Cottage Crisis Stabilization and Shelter - North Homes Children and Family Services

Provides a safe placement setting for males and females, 10-17 years old. Upon admission each child receives a health screening by a registered nurse, a case manager to assist the family with additional options and direction, and a treatment plan for stabilization and discharge. Crisis Stabilization Unit - Woodland Centers

Adults and youth ages 6 - 17 who are experiencing a mental health crisis. Provides a supportive environment for individuals needing a safe place to recover and stabilize during a crisis. Individuals staying at Crisis Stabilization will meet with a qualified mental health professional, mental health practitioners, and nursing staff during their stay. Services offered include skills training in individual and group settings, evaluation, stabilization, and referrals. Average length of stay is 1-10 days.

Family Response Services - Hennepin County Human Services - Social Services

Hennepin County works with community agencies to deliver the Family Response service. Family Response can help, with immediate, in-person support and stabilization for children, youth, and their parents/caregivers. The model's goal is to reduce hospitalizations and other out-of-home placements by providing families with immediate, upstream support. There are no specific criteria required to initiate services, but concerns may include children and youth who are: - Experiencing worry and concern - Feeling down or less engaged - Having a hard time with a family member or change in the home - Having issues at school - Showing aggression or anger When a call is made to Family Response, staff will gather basic information and send a family response team to the family's location within one hour. The Family Response team will: - De-escalate and address the immediate concern - Engage and support the family over the next 72 hours - Connect families to support, including an optional 8-week stabilization service. Stabilization services can provide healthy, safe behaviors, and help the child manage their daily activities.

School Linked Mental Health - Central Minnesota Mental Health Center - CMMHC

Works in partnership with school districts to provide mental health services for students through 12th grade to remove obstacles to treatment. Providing mental health services directly in the school setting increases access to children and adolescents. It allows collaboration with schools and community support systems to improve outcomes for those with mental health concerns. Mental health professionals engage in school interventions and design toolboxes to use across classrooms, embedding these resources in the school to help reduce referrals. Can see kids throughout the school day and coordinate with school staff to meet the needs of what is happening in the school. Children and Family Services - Hmong American Partnership

21st Century Community Learning Centers: In partnership with the Minnesota Department of Education, HAP works with Community School of Excellence (CSE) and Prairie Seeds Academy (PSA) to after school academic and enrichment activities that include: the arts, music, crafts, leadership and personal develeopment, STEM classes, career exploration, and additional services. Ascend: Works with victims and at-risk youth to address the severe harm caused by their explotation and move them towards recovery and independence Hmoob Toj Sjab: A Montesorri school in which children learn all the foundation necessities for survival in their enviroment. Children learn all the basic building blocks that make up the world within and around them. They build awareness in identifying colors, shapes, textures, and dimensions; accomplish simple chores; develop manners such as grace and courtesy; learn to care for themselves and their enviroment; and build reading, writing, and math skills. In-Home Parenting: In-home parenting services and support to families referred by Ramsey County. Learn and Live: Health Education Programs: Education and outreach to the Hmong Community for breast and cervical cancer awareness and screening. Pathways to Eliminate Addiction: Adolescent and Adult Chemical Health Support program for Hmong individuals. The goal of the program is to reduce drug and alcohol use in the Hmong community by working with individuals to achieve or sustain abstinence, improve health, wellness and a quality of life. Project Grow: After-school program for ages 14 - 21, training students in exploring leadership, agriculture, college and career, and conservation. Saint Paul Promise Neighborhood Hmong Navigator: A HAP family navigator located at Jackson Elementary works at the school's Family Center to help connect low income families with minor children to community resources. Transcend: Crises intervention, social supports, safety planning, and legal advocacy for Hmong and Karen women who are experiencing domestic violence. YouthLEAD: Career education and paid internships for SE Asians ages 16 - 24. Must reside in Ramsey County.
