Transportation Services - Aging Services for Communities
Provides transportation services to older adults over the age of 65 by trained volunteers. Services include:
- Door-to-door service
- Individuals on Blue Plus or UCare with Medicaid benefits are eligible for rides
- Transportation to older adults who are not on Medical Assistance. Transportation services include medical appointments.
- Transportation to older adults who are on Medical Assistance. Medical appointments can be anywhere in Minnesota. Clients must be able to get in and out of the vehicle on their own.
- Transportation is not wheelchair accessible

Consultation Services for Community First Services and Supports (CFSS) - Arc Northland
Community First Services and Supports (CFSS) is a Minnesota health care program that offers flexible options to meet the unique needs of people. CFSS allows people greater independence in their homes and communities. CFSS replaces personal care assistance (PCA) and the Consumer Support Grant (CSG).
CFSS covers the same main services as PCA:
- Activities of daily living, such as eating, bathing, grooming and transferring
- Health-related tasks
- Instrumental activities of daily living, such as shopping, cooking, laundry, and assistance with self-administered medications
- Observation and redirection of behavior.

Conducts child protection investigations on all neglect and abuse complaints within the Band's jurisdiction. The program provides ongoing child welfare case management and prevention services to Band members, first-generation descendants, and American Indians within the Band's designated service area. The Indian Child Welfare program monitors the welfare of all Band children and first-generation descendants outside the Band's designated service area under the Indian Child Welfare Act.
Foster Care:
Licenses and trains foster care providers within the Band's designated service area. Any American Indian or community member with strong ties to the Mille Lacs Band may apply to become a licensed foster care provider.
Independent Living and Life Skills program:
Provides child welfare support services to Band members, first-generation descendants, and American Indians who are 14 - 21 years old and live within the designated service area of the Band. It also provides independent living assistance to Band members, first-generation descendants, and American Indians who are 16 - 21 years old and live within the Band's designated service area.

Medicare Savings Program - Polk County Social Services
Medicare Savings Programs have four sub-programs:
- Qualified Disabled and Working Individuals (QDWI): Helps pay for Part A premiums only. Monthly income limits for individuals and married couples apply.
- Qualified Medicare Beneficiary (QMB): Helps pay for Part A and B premiums as well as deductibles, coinsurance, and copayments. Monthly income limits for individuals and married couples apply.
- Qualifying Individual (QI): Helps pay for Part B premiums only. Monthly income limits for individuals and married couples apply.
- Specified Low-Income Medicare Beneficiary (SLMB): Helps pay for Part B premiums only. Monthly income limits and countable resource limits apply for individuals and married couples
