Targeted Case Management: Helps adults with a serious and persistent mental illness gain access to needed medical, social, educational, vocational, financial, and other necessary services as they relate to the recipient's mental health needs. Also collaborates with two ACT Teams in Duluth and one in Virginia within Saint Louis County. Civil Commitment: Involuntary commitment for persons determined by the courts that are warranted. Works with courts, corporate counsel, law enforcement agencies, guardians, and families to see that people who are committed receive the services they need in the least restrictive setting possible.

The assessment takes a person-centered approach to ensure that services meet each individual's strengths, goals, preferences, and assessed needs. The assessment replaces the following assessment tools: - Developmental Disability Screening - Long-term Care Consultation Assessment - Personal Care Assistance Assessment - Home Care Nursing Assessment (to be included in a future enhancement) The assessment determines eligibility for publicly funded programs and services including: - Alternative Care Program - Brain Injury Waiver - Community Alternative Care Waiver - Community Alternatives for Disabled Individuals Waiver - Developmental Disability Waiver - Elderly Waiver - Case management for people with developmental disabilities - Semi-Independent Living Services NOTE: Community or in-person screening is required in some situations to determine eligibility for waiver services

Provides behavioral and mental health services for children and teens such as: - Behavioral Health Services: Provides resources for children who have a full range of mental health and chemical dependency issues. Resources for psychologists, therapists, counselors and psychiatrists services are provided to children in the hospital or at a clinic closer to individuals home. - Birth to Three Program: Mental health services for children who are at-risk for experiencing toxic stress. Stressors such as, multiple hospitalizations, chronic illness, stressful transitions, physical or emotional abuse or neglect, and for parents who are struggling with own stressors. - Dual Diagnosis Inpatient Program: Offers short-term crisis care for children and teens to stabilize their mental or emotional condition. Treatments plans designed based on child or teens needs and medical history. - Family Involvement: Family members are expected to participate in the assessment process and attend regular family therapy sessions. Participation in family therapy will help all family members better understand and cope with the effect of emotional and behavioral disturbance.

Hotline for reporting foodborne and waterborne illnesses within Minnesota. Hotline staff will collect information about symptoms, food eaten and when it was eaten during the 4 days before the illness began. They will also ask about drinking well water or if an individual went swimming in the two weeks before the illness. All information is private and will not be shared with anyone outside the public health system without permission. Callers may be asked to provide a stool sample to determine what caused the illness. The report will be forwarded to the environmental health staff responsible for inspecting restaurants and grocery stores.
