Coordination of behavioral resources. Goal is to reduce the need for hospitalizations and allow people to successfully remain in the community. Services: - Preventative behavioral interventions, assessments, and crisis support planning - Technical assistance and training to families, day programs, schools, residential programs, and care providers serving individuals with intellectual disabilities and/or related conditions as well as individuals receiving services though a CADI waiver - Emergency telephone consultation - Coordination of transitional housing within the Metro Crisis Coordination Program (MCCP) crisis provider system - Support to reduce behavioral crisis that adversely affect the residential, work, or educational placements of people with intellectual disabilities and/or related conditions

Services include but are not limited to:? - Access to medical support and follow-up on medical directives - Assistance in applying for publicly funded programs - Connecting to food support programs - Employment and volunteer readiness and placement - Home visiting and home management skills - Housing assistance - Ongoing one-on-one daily living skills counseling and general support - Safety planning - Shopping, budgeting, financial, and application assistance - Support with legal issues, daycare, school, and landlord/tenant issues - Waivered services

Program for adults who are physically and/or mentally disabled. We do include some students in their transition years. Program runs primarily September through June: with an activity in July and August. Weekly program provides an opportunity for social interaction with peers. Classes include subjects like: crafts, music, arts, theatre, cooking, games, dances, nutrition and photography.
