Community Connections - Southside Services I, Inc.

Provides highly individualized crisis intervention, advocacy, and navigation services that connects adults to community support systems that enable them to continue to live independently. Staff work directly with participants to identify and resolve barriers and typically accompany participants on initial meetings and appointments when facilitating connections. Group Homes - ACR Homes

Operates group homes located in 58 sites throughout the five-county suburban Twin Cities: - 2 - 6 person homes in suburban locations for those with disabilities; typical ratio of two staff for four residents - ACR keeps residents actively engaged in the community with activities that are meaningful to clients - Each client typically has their own bedroom and the rest of the home is shared - Each home has a live-in supervisor which helps to keep knowledgeable communication with loved ones as a component of care - Most homes are wheelchair accessible, and have an accessible van. Will make and take individuals to doctor's appointments and other activities. - Supports clients with g/j tubes, ostomies, catheters, oxygen, nebulizers, vest treatments, individuals who require range-of-motion and other therapies such as standing frames, braces, and mechanical transfers.
