Helps older adults manage chronic conditions to improve quality of life by offering: Living Well with Chronic Conditions: A Chronic Disease Self-Management Program provides tools to helps people manage chronic conditions such as diabetes and heart disease through a six-week educational workshop. The workshop meets for two hours each week and is held at a variety of locations. Familiy and friend caregivers are welcome to particpate.

Provides sleep studies by a trained sleep technologist to monitor brainwaves, eye movement, breathing, blood oxygen levels, heart rate, and muscle activity while patients sleep in a private study room. A sleep disorder diagnosis and treatment are provided through electroencephalograms done in the sleep lab.
