Caregiver Support and Respite - Aging Services for Communities

- Respite Care: Respite care is provided by trained volunteers to provide caregivers the support they need. Schedules are arranged for volunteer visits. - Education sessions on topics such as conflict resolution between the caregiver and the rest of the family, personal property of the person being cared for, Dementia/Alzheimer's, stroke, and other related conditions

Physical Medicine and Rehabilitation Services - Allina Health - Annandale Clinic

Focuses on diagnosing and treating problems that affect the muscles, bones and joints, as well as the nervous system. Treatment is provided for: - - Amputations - - Back pain and neck pain - - Brain injuries - - Cerebral palsy - - Concussions - - Fibromyalgia and other chronic pain conditions - - Joint and muscle injections - - Movement disorders - - Musculoskeletal disorders - - Multiple sclerosis - - Pain problems following trauma and surgeries - - Performing arts injuries (musicians, dancers and gymnasts) - - Post-polio - - Spasticity and other muscle tone abnormalities - - Spinal cord injury, paraplegia and quadriplegia - - Spine care - - Sports injuries - - Stroke A physiatrist conducts physical exams, prescribes healing therapies and develops treatment plans. A physiatrist can also prescribe spine and large joint injections as needed.

Resource Facilitation - Minnesota Brain Injury Alliance

Free, two-year telephone support program provides education and connection to supports and services to assist people throughout Minnesota in navigating life after brain injury. Participants receive scheduled calls over two years to help problem-solve issues and identify resources to help them transition back to family life, work, school, and the community while achieving the greatest level of independence possible. Referrals also provided to a statewide network of support groups for persons with brain injury, their families and friends.

Case Management and Care Coordination - Minnesota Brain Injury Alliance

Provides case management services for persons who have qualified for waivers. Offers case management for Community Access for Disability Inclusion (CADI), Brain Injury (BI), Developmental Disabilities (DD), Community Alternative Care (CAC) waivers, and Rule 185 case management.? ?Care Coordination helps persons enrolled in Medical Assistance (MA) managed care programs. Helps set health goals, coordinate care, and access services like transportation and referrals for medical, mental, and dental health needs.
