Transitional Services: Individuals receive damage deposits, essential furniture, household supplies, and moving assistance to help transition into their own home in the community. ?Nursing Home Relocation (NHR): Assists people who wish to move out of a licensed facility into stable community housing. Staff will research options based on the individual's preferences, then create a timeline, relocation plan, help identify needed community support, and manage all aspects for a seamless move. - Also helps secure medical, social services, and support needed to help an individual live as independently as possible. Those services can include hiring a personal care attendant (PCA) who can help with essential daily activities like bathing, dressing, and eating.

Services include: - Assistance with navigating insurance coverage options of patients and helping resolve issues with their insurance company. Secure access to care if individuals are uninsured. - Ensure access to care and benefits such as Social Security Disability (SSI or SSDI) or state disability benefits, employer-sponsored benefits, sick leave, short-term and long-term disability, and COBRA. - Finding resources for financial aid and/or prescription drug assistance programs - Help with applying for programs that pay for basic needs expenses - Secure online portal to connect with a ZERO360 case manager. Case managers can work with individuals to help negotiate free or reduced-cost treatment or set up payment plans.

Helps patients pay their bills while they undergo treatment for cancer. Pays essential living expenses (car repair, electricity, heat, medical, mortgage, phone, rent, and water) so patients can focus on getting well. Maximum payment for patients receiving treatment is $1,500/year. Prefers to pay larger bills instead of multiple smaller bills. If a bill exceeds $1,500, the organization will pay $1,500 towards the bill. Financial assistance is provided directly to the creditor, not the individual requesting it.
