Transitional Residential Substance Use Disorder Services - Sunrise Care Facility - Spring Grove

Sunrise Care Facilities are designated by the State of Minnesota as Rate 2 Housing with Special Services. The men's and women's facilities have a capacity of 10 residents each. Residents work with staff on day-to-day tasks, utilizing copying skills and developing goals to eventually live on their own. Staff assist residents with completing medical, insurance, funding, and legal documents. Residents are encouraged to participate in community affairs and to seek employment when appropriate. Support for residents to attend AA meetings and seek sponsorship is provided including with transportation to meetings. Weekend passes and family visits are encouraged. Smoking is permitted outside. No mood-altering chemicals are allowed on the premises.

Guardianship Options - Lutheran Social Service of Minnesota - Metro

LSS Guardianship Option services include: Guardianship or Conservatorship: Manages the health and care of incapacitated individuals in the least restrictive manner possible and advocates in their best interest. Guardians and Conservators are appointed by the court. If an individual has no family or friends to assume the responsibility, LSS my be appointed. LSS staff gather information from individuals, caregivers and medical personnel to make medical and care decisions. Personal Representative of the Estate: Following court appointment, LSS completes probate requirements and handles disposition of assets according to the individual's wishes. Power of Attorney: Through a legally executed power of attorney, LSS is authorized to assist individuals in managing income and assets. Services may include check writing, checkbook balancing, assistance with financial and medical forms, assistance with retrofitting, maintenance, preparation for sale, the sale or purchase of property. Pooled Trust: Protects the public benefits of people with disabilities of any age while maintaining resources that will be available for their special or supplemental needs. The creation of a pooled trust moves assets from being countable for purposes of Medicaid and Social Security eligibility to being non-countable assets. Care Coordinator: Partners in decision making with individuals who may be alone and who may need help with contracting of repairs or services in the home, scheduling of appointments and making payments to providers.

Alcohol and Chemical Dependency Treatment - Northland Recovery Center - Grand Rapids

Detoxification: - Removes toxic or harmful chemicals from the body. - Individuals are assisted by specially trained medical and counseling staff. - Available 24 hours a day, 365 days per year. Inpatient Treatment: - Provides intensive therapy in all aspects of recovery for those who are unable to maintain abstinence during outpatient treatment. - Residential stay is followed by an individualized schedule of outpatient treatment during day or evening hours. - Length of treatment varies according to the length of the residential stay, but typically averages 4-6 weeks for combined residential and outpatient treatment. Outpatient Treatment: - Intensive, conveniently structured treatment program for those individuals who are able to abstain from use of alcohol and other drugs while involved in the treatment process. - Treatment may be arranged during the day, evening, or a combination according to need. Aftercare and Relapse: - Provides support and stability for ongoing recovery of individuals who have completed primary treatment. - Aftercare groups (both men's and women's) focus on issues such as coping skills, securing ongoing support, family and marital dynamics, work pressures, socializing, spirituality, relaxation, and stress management. - May be recommended for those considered "high risk" for returning to alcohol or other drug use. - Builds awareness of a person's personal relapse process. - Explores old patterns and new choices to stop the cycle from repeating. Assessment Unit - Leech Lake Band of Ojibwe

- Provides Rule 25 Chemical Health Assessments for Native American people living within the boundaries of Leech Lake Reservation. - Provides assessments for ages 11 through adult. - Referrals are received through self-referrals; courts; probation; Child Welfare; and sometimes through detox centers, hospitals and schools. - Works with approximately 45-50 providers statewide, as well as referrals to the various Leech Lake CD programs. - Clients are referred to Inpatient Treatment, Outpatient Treatment, Aftercare and Relapse Prevention programs, and Halfway Houses. - Collaborates with treatment centers, social workers, probation officers, hospitals and schools to make the best possible plans for clients to obtain and maintain sobriety. - Priority assessments for pregnant women and adolescents. - Civil commitments for clients who are very ill due to chemical/alcohol use. - Data privacy and confidentiality are strictly adhered to according to policy.

Project CHILD - Hennepin County Human Services - Social Services

Project CHILD is for pregnant people ages 16 and older or who have been reported to Child Protection for controlled substance use anytime during their pregnancy. The program can also support those already in active recovery to reduce the risk of relapse during pregnancy. Services offered: - Help with basic needs including transportation - Substance use assessments, treatment services, case management, and counseling for up to six months after delivery - Peer recovery support specialist services - people with a lived experience in substance use and recovery - Prenatal care referrals, health assessments, and individualized care planning with a nurse - Prenatal and postnatal support groups. Parent(s) learn about parenting and baby development while building community with other parents. Also, offer support for all caregivers. - Project CHILD collaborates with many county and community-based resources for services. NOTE: Pregnant women under the age of 16 should be reported to the Child Protection Screening Intake line.
