Adult Mental Health and Recovery Services include the following programs: Assertive Community Treatment: Assistsadults experiencing severe and persistent mental illness stabilize, recover and stay in community. One of the only programs in the nation that provides multilingual and multicultural services to members of Southeast Asian communities. Assertive Community Treatment services include: - Community-based psychiatric care - A multidisciplinary team including psychiatrists, mental health therapists, nurses, case managers, chemical dependency specialists, vocational specialists and certified peer support staff. - Multilingual, multicultural staff representative of the Southeast Asian communities served: Hmong, Karen, Vietnamese and Cambodian. - Help with navigating healthcare systems, psychiatric care/medication management, mental health rehabilitation services, accessing housing, employment, transportation services, and family support. - Services are provided in office, home or community settings to ensure each person we serve is comfortable and able to access the support they need. Serves Southeast Asian adults 18 years and older who have a severe and persistent mental illness (SPMI) diagnosis and are qualified for Medical Assistance. Assertive Community Treatment is most beneficial for adults who have repeated hospital admissions, chronic episodes of homelessness, or who have been unsuccessfully served by traditional mental and chemical health care options. Adult Recovery Services: Helps individuals and families experiencing mental health and substance use issues by building on each person's strength and resilience. Staff have experience in co-occurring mental health and substance use concerns ranging from low to high intensity. Specializes in assessing, counseling and treating adults 18+ and their families with group treatment available in English, Hmong and Karen languages. Provides an integrated care team of therapists, nurses, alcohol and drug counselors, psychologists, psychiatrists, nurse practitioners, care coordinators, peer specialists and case managers work together using a holistic, trauma informed and whole family approach. Adult Rehabilitative Mental Health Services: Helps individuals manage their mental health symptoms and achieve their life goals. These services are provided in one's home or community, and help address the need to function in ones day-to-day lives: achieving physical and emotional health, reaching education or employment goals, improving social and family relationships, using community resources, being active in the community and living independently. Outpatient Therapy: Providesdiagnostic assessments, individual and family therapy, treatment planning and referrals for other services Peer Services: Provides certified peer specialists who have experienced their own mental health and addiction issues. These trained specialists work alongside professional staff to provide additional encouragement and personal support. Peer specialists help people build coping skills and access local resources to improve individuals health. Targeted Mental Health Case Management: Individuals 18 years and older who have a severe and persistent mental illness diagnosis can qualify for targeted mental health case management services. Each individual is assigned one case manager who understands their challenges and needs. Case managers can coordinate mental health, physical health, and other resources to provide ongoing support so that each individual can increase their overall well-being.

The Community Health Workers Peer Network program reaches a statewide group of Community Health Workers serving diverse communities. Provides bi-monthly professional development trainings for Community Health Workers in the Peer Network including employers and key stakeholders. Trainings are recorded, archived, and posted online.

Financial Education program which offers individuals the opportunity to learn the basics of creating a budget, banking, building credit, and the importance of saving. Services include: - Financial Coach: Provides one-on-one service to help individuals create a budget, review credit reports, work on financial goal planning, and learn how to move toward financial stability and independence - Home Stretch: Partnership with MN Housing and MN Homeownership Center; an 8-hour class that walks through all of the steps of purchasing a home. Individuals will hear from realty, lending, and home insurance professionals.

A variety of adult education programs are available through Neighborhood House with the goal of helping individuals develop the skills necessary for gaining economic self-sufficiency. Career Training Programs: In partnership with Avivo, training is offered on workplace essential skills, and various sector-specific careers such as maintenance and custodial, medical office support, and more. College Readiness: The primary focus of this class is to prepare students for the Accuplacer test, which colleges use to determine if students are ready for courses at that level or if they would benefit from remedial English or math classes. The goal is to help students pass this test so that they can enroll directly in credit-bearing classes and avoid expensive remedial courses. Students will also learn the soft skills necessary to succeed in college. Computer Classes: Students learn how to use the internet, email, and Microsoft Office programs. Every other day of computer class is run as an open lab where students can work on any computer-related project and get support from staff and volunteers. Students work toward gaining a Northstar Digital Literacy Assessment certificate, a credential recognized throughout the state. English Language Civics: Funded by the Minnesota Department of Education, civics classes teach English skills through a focus on US citizenship, community engagement, Minnesota culture and history, and job skills. Students in these classes all have a goal of eventually obtaining employment or improving their current employment situation. Many students in this class also volunteer at Neighborhood House in order to gain job skills. Offers a course to prepare individuals for the citizenship exam. English Language Learner (ELL) Program: Classes teach students reading, writing, speaking, and listening skills through the context of life skills. Students learn about health, housing, work, food, money, and many other everyday topics. After students complete the Advanced ELL class, they are able to join the GED or College Readiness classes. GED Exam Preparation: Classes are designed to help students prepare for the test in an engaging classroom. These classes are a combination of US-born students who did not complete high school and English Language Learners who have completed English classes and need a high school credential for work or college.
