High Rise Social Services - Volunteers of America Minnesota and Wisconsin

Provides older adults and individuals with disabilities with services and supports that help them thrive while living independently and remain safely in their own homes. Community Health Workers and Social Workers serve older adults and individuals with disabilities in 40 Minneapolis Public Housing Authority high-rises and Village at Franklin Station affordable housing property in Minneapolis. Services may include assistance with self-managing chronic health conditions, improve connections to community services, navigate and access health systems, address transportation barriers, and attend doctor appointments to assist with translation and advocacy. Social Workers help individuals navigate complex systems, such as economic assistance, physical and mental health systems, financial concerns, housing needs, and personal crisis. Bilingual Somali team members are available to meet the language and specialized cultural needs of Somali residents as well as remote translation services to meet the language needs of all residents.

- Mental Health Case Management provides individualized support to children with severe emotional disturbances (SED) and adults with severe and persistent mental illness (SPMI). - Mental health targeted case managers use a person-centered approach to assess, plan, refer, coordinate, and assist the person in gaining access to a variety of services including mental health, medical, social, educational, housing, and vocational. - Provides culturally specific services for children or families who are Deaf, Deafblind, or Hard of Hearing by mental health targeted case managers who are fluent in American Sign Language.

Meals on Wheels - Three Rivers Community Action

Provides a hot, nutritious noon meal to older adults, adults with disabilities, or adults who are terminally ill and with limited mobility who are unable to prepare their own meals. Meals are provided by local caterers and delivered by volunteers. Program Coordinators will meet individually with client to discuss nutrition and other services available to aid them in remaining independent. Volunteer opportunities available to drive for Meals on Wheels.
