Offers a long-term assistance program that helps families facing unemployment, unstable housing, and lack of education and assists in finding the need. The goal of this program is to support families to become self-sufficient. Families are assigned a case manager who partners with them to set and accomplish goals.

Outreach Services - Milestones - Saint Cloud

Provides outreach services and support to families of young children and the adults that care for them, including licensed family child care providers, child care centers, and family friend and neighbor caregivers. Programs include: - Connecting to a childcare liaison to assist families with the process of securing child care that meets the family's needs, including county and enrollment forms - Resources, support, and training to family friends, and neighbors who are unlicensed caregivers to improve the quality of care provided to children - Car seat education, distribution, and installation for families with qualifying insurance. Car seat education classes are also provided in Somali and can be translated into other languages as needed. - Shared Services Alliances: Designed to work in partnership with Early Child Care programs to offer the services needed to improve long-term financial strength and manage capacity, along with the abilitiy to provide care and services.

Provides community health programs such as: - Child and Teen Checkup Outreach: Helps families find a doctor, dentist, psychologist, and transportation - Disease and Prevention Control: Monitor and investigate potential disease processes through adult/child immunizations, communicable disease prevention programs, emergency preparedness plans, surveillance of the county's environment, and tuberculosis screening - Environmental Health: Help protect the residents from environmental health hazards through programs to reduce exposure to health risks, such as food and waterborne diseases, radiation, occupational health hazards, and public health nuisances. Provides radon, lead, and well water education. - Follow-Along Program: Provides parents with information about their child's development during infancy and early childhood through mailing questionnaires - Health Promotion: Provides presentations and/or materials on health topics for small and large groups. Growing Kinship Connection (GKC) - Family Rise Together

Program aims to reduce risk factors and increases protective factors for children and youth by: - Connecting children with positive adults (Kin caregivers) - Educating and providing resources to relatives to navigate the complex system - Increase formal and informal placement with relatives and paternal family - Provide culturally specific training, such as 24/7 Dad's AM/PM curriculum, etc. - Provide equitable case management services Services including: - Education: Helps educate and empower families to better navigate the complexities of the system. Provides resources that focus on navigating the court system and becoming a better parent, with an emphasis on fathers/father figures. - Family Mentorship: Program replaces traditional case management by recognizing the oppression built into "managing family affairs". Mentors work with alongside families to educate, provide resources (i.e. swag bags, resource guides), and nurture familial bonds for improving the well-being of children and families. This service is currently offered in Saint Louis County. - National Relative Search: Offers a relative search service to GKC families actively working with. This allows the family mentor to identify possible relatives across state lines to determine possible placement options and nurture familial connections. - Support Groups: Provides a peer-to-peer support, for fathers and male care givers in south Saint Louis County
