Hennepin County residents who are experiencing homelessness can receive housing case management assistance through the Homeless to Housing program. The primary focus of the Homeless to Housing team is to fill system gaps by offering case management to people who otherwise lack support and access to resources by helping them secure permanent housing. The Homeless to Housing teams can help single adults, families, youth, and veterans who are staying in shelter or those living unsheltered with the following: 1. Referral 2. Intake 3. Goal planning 4. Barrier busting 5. Housing identification and move in 6. Stabilization and referral to long-term support

Licensed group homes provide a nurturing and structured living environment and intensive support for youth ages 12 to 18 whose primary issues stem from family conflict, behavior problems, school, or social difficulties, or criminal activity. Services include: - 7-day, 29-day, and 90-day structured programming available - Assists referring agency in identifying and addressing issues contributing to individual and family dysfunction - Provides educational services on site, with a licensed EBD teacher - Provides observational report to the referring agency - Upon request, psychological assessment completed by a licensed clinical psychologist

Provides rapid re-housing services where members of the community open their homes to host a youth or young adult in Anoka County who are experiencing homelessness. Provides case management services to help facilitate transitions, minimize barriers, and help youth and young adults set and achieve their goals.

Provides basic needs such as hot meals, grab and go foods, clothing, laundry facilities, a shower, and more. Additionally, gives access to case managers on-site to help connect youth to education, employment opportunities, health and wellness programs, stable housing, and more. Also provides a 12-unit transitional housing facility in Coon Rapids that offers supportive services to young people who are at risk of or experiencing homelessness.

Works with individuals sleeping outside or in substandard places and assists them to get into permanent housing. Provides a non-emergency response to homeless concerns in public settings with a mobile outreach team able to visit individuals in the community. Can help address immediate basic needs for safety and survival outside with items such as food, personal grooming supplies, outdoor gear, clothing, and shelter referrals. Low income and individuals experiencing homelessness can pick up a free Handbook of the Streets from 9 am - 4 pm Monday - Friday or download the PDF from the website.

Program designed to meet the specific educational and employment needs of youth. Services include: Education: Assists with obtaining a social security card, birth certificate, and/or state ID, application assistance for food stamps, Medical Assistance, or financial aid, advocating at school, connecting with GED tutors, and preparing for college applications. Financial assistance for obtaining vital documents is available. Employment: Assists all youth with online job searches, applications, resumes, and practice interviewing. Providing work appropriate clothing, transportation in the form of bus passes and bus tokens to get to and from work and for interviews and Lyft/Uber for a job interview, and referrals to partner organizations to find other employment resources. YouthLink partners with HIRED/Goodwill Easter Seals.

Temporary emergency lodging for individuals and families who are homeless including: - Dinner - Sleeping accommodations - Light breakfast - Lunch - Showers - Guests must be assessed by the GRACE House Director or supervisor prior to acceptance at GRACE House - Guests will not be accepted after 8 pm - Overnight accommodations may be needed until assessment can be done - Day services available for registered guests - To volunteer please contact Grace House at (218)326-2790. Resource Centers: During designated drop-in hours, YMCA Resource Centers are staffed by an Outreach Worker who can help youth, ages 12 - 24, with basic needs support such as food, clothes, hygiene items, household supplies, parenting items, and more. Also during drop-in hours, youth can stop in to get their coordinated entry assessment completed, help finding shelter, screened for prevention services, or access other YMCA resources/services. Four locations: Lion's Resource Center/Northeast Minneapolis, Emma's Place in Coon Rapids, Maplewood, and Saint Paul Midway Safe StaY Emergency Shelter: Moves youth off the streets into a safe space and provides supportive services. School and Street Outreach: Youth Outreach Workers provide services for youth and young adults who are homeless or at risk of homelessness at schools and out in the community to meet immediate needs and connect them to community resources. Youth Resource Line: A free message line for local youth age 12 - 24 who are experiencing homelessness, or are at risk. Assistance with resources and community referrals.

Provides emergency rental assistance with goal-oriented case management to address the immediate housing crisis, prevent homelessness, and promote long-term housing stability by addressing barriers related to homelessness. Supportive services may include goal setting and stability planning, budgeting support, employment search, transportation assistance, childcare and benefits navigation, help identifying healthcare providers, and much more. Provides rapid re-housing services where members of the community open their homes to host a youth or young adult in Anoka County who are experiencing homelessness. Provides case management services to help facilitate transitions, minimize barriers, and help youth and young adults set and achieve their goals.
Provides a host home program and supportive services for youth who are marginalized and identify as LGBTQI+ in the Twin Cities metro area. It can take 3 - 6 months for someone to move into a host home. Program provides support for up to 1 year.
