Outreach and Support Services - Bridge for Youth, The

- Outreach program to youth in crisis, connecting them to resources and services, and promoting their safety, stability, and full potential. - Mobile Youth Outreach Center: Provides virtual support and 1:1 in-person mobile intervention to meet immediate basic needs including health food, gender affirming clothing, resources for harm reduction and safety planning, assessments and referrals, and case management. Locate mobile bus sites and locations throughout the Twin Cities by accessing the online schedule or the YSN mobile app.

Drop-In Center - HOPE 4 Youth

Provides basic needs such as hot meals, grab and go foods, clothing, laundry facilities, a shower, and more. Additionally, gives access to case managers on-site to help connect youth to education, employment opportunities, health and wellness programs, stable housing, and more. Also provides a 12-unit transitional housing facility in Coon Rapids that offers supportive services to young people who are at risk of or experiencing homelessness.

Homeless Services - Range Mental Health Center

Provides homeless outreach and case management services to adults with serious and persistent mental illness. Offers housing options including crisis beds, rental assistance, and permanent housing. Outreach, case management, and rental assistance are also available to homeless youth under the age of 24. NOTE: Single adults and youth must get a pre-screen through United Way 211 to access this program. The pre-screen information is provided to Saint Louis County Housing Assessment Team (SLCHAT).

Street Outreach Program - Agate Housing and Services

Works with individuals sleeping outside or in substandard places and assists them to get into permanent housing. Provides a non-emergency response to homeless concerns in public settings with a mobile outreach team able to visit individuals in the community. Can help address immediate basic needs for safety and survival outside with items such as food, personal grooming supplies, outdoor gear, clothing, and shelter referrals. Low income and individuals experiencing homelessness can pick up a free Handbook of the Streets from 9 am - 4 pm Monday - Friday or download the PDF from the website.

Hennepin County residents who are experiencing homelessness can receive housing case management assistance through the Homeless to Housing program. The primary focus of the Homeless to Housing team is to fill system gaps by offering case management to people who otherwise lack support and access to resources by helping them secure permanent housing. The Homeless to Housing teams can help single adults, families, youth, and veterans who are staying in shelter or those living unsheltered with the following: 1. Referral 2. Intake 3. Northern Lights Drop In Center - Servants of Shelter - International Falls

Emergency Shelter and Day Center for people experiencing homelessness. Servants of Shelter operates a 5 apartment unit Family Shelter. Shelter guests receive Case Management and Housing Resource Navigation services during their 30-90 day stay. SOS operates a Day Center with laundry, showers, a communal kitchen, hygiene items, and more. SOS also operates two Permanent Supportive Housing programs for people living with disabilities who are also experiencing homelessness.
