- Childhood and adult immunizations

- Recommended age-appropriate vaccinations

Services provided:

- Investigates and treats Tuberculosis (TB) - Provides control function of managing communicable disease outbreaks - Schedules immunization clinics for child/adolescent/adult immunizations. - Screens and supports refugees and immigrants Vaccines provided under the Minnesota Vaccines for Children Program and the Uninsured /Underinsured Adult Vaccine program:

- Adults: Tetanus, pneumonia, HPV (Gardasil), MMR (measles, mumps, rubella), Hepatitis A, Hepatitis B, Influenza, chickenpox, and shingles vaccinations. Any adult aged 19 or older who is underinsured or uninsured can receive these immunizations.

​- Children: All required vaccines for day care, Headstart, and school (preschool - high school). Plus recommended vaccines including Hepatitis A, Human Papillomavirus (HPV), Rotavirus, and Influenza.

Provides the following services and information to prevent the spread of diseases:

- Assists with and conducts infectious disease investigations

- Communicable disease prevention, education, and follow-up to control the spread of disease

- Coordinates infectious disease investigations with the Minnesota Department of Health

- Offers immunizations and information on immunization schedules and records

- Provides information on various communicable diseases and conditions

Monitors diseases and promotes a healthy community. Services include:

- Adult and children's vaccinations and information. Vaccinations are offered at the Horizon Public Health Office in Alexandria.

- Community education resources to protect and prevent diseases from insects, common infections, lead poisoning, and viruses

- Safe drinking water testing and helps address common concerns, including arsenic, coliform bacteria, lead, manganese, and nitrate and support treatment of contaminants in private wells

- Substance misuse prevention resources

- Adult and children's vaccinations and information. Vaccinations are offered at the Horizon Public Health Office in Alexandria.

- Community education resources to protect and prevent diseases from insects, common infections, lead poisoning, and viruses

- Strives to help prevent the chronic disease risk factors of tobacco use and exposure - Preventing poor nutrition and physical inactivity - Create sustainable, systemic changes in schools, work sites, communities and health care organizations that make it easier for Minnesotans to incorporate healthy behaviors into their daily lives - Focuses on policy, systems, and environmental changes where we live, work, learn, and play

Provides information on: - Heart Disease - Diabetes - Nutrition - Lyme Disease - Cancer - AIDS/HIV and STD's - Family Planning - Smoking/Tobacco - Other health related topics

Health Education and Promotion Programs attempt to maintain and improve the health of our county's residents through many types of programs such as: Group Programs about health concerns such as: - Nutrition - Healthy lifestyles - Hypertension - Smoking - AIDS, etc - Health Related Programs for schools and youth organizations - Assistance and direction with the formation of needed support groups - Diabetes Education is provided by one of our staff nurses who is a certified diabetes educator - Blood Pressure and Cholesterol Screening, education and referral for on-site groups and work places - AIDS information and support, call the AIDS Hotline at (800)248- AIDS - For local education and/or promotion, call County Health at (218)283-7070 - Up to date health information and audiovisual materials are available free of charge to individuals, community groups, schools - Public Awareness of Health Resources throughout the county through publicity, programs and health information - What's Up? Listen to KGHS Radio Wednesday afternoons & Saturday mornings - Labor and Delivery class training is provided through the Rainy Lake Medical Center Hospital Campus - The Maternal and Child Health Special Project seeks to reduce infant mortality and disability and to improve the health of the mothers and babies. Home visits are made to pregnant women and mothers and infants by a skilled maternal and child health nurse. Education and support services to reduce maternal risk-taking behaviors and improve parenting skills are provided through individual counseling, group education, prepared childbirth classes, special classes for students on healthy lifestyles, and the distribution of printed materials and newsletters - Family Planning Services are available at Koochiching Family Planning Clinic

- Collaborative wellness initiatives with community partners, schools, workplaces, and organizations to promote healthy lifestyles and prevent chronic disease (e.g., tobacco-free initiatives, physical activity promotion)

- Community health assessment and planning to identify local health needs and guide community health improvement efforts

- Communicable disease investigation, follow-up, and control measures to prevent the spread of infectious diseases

Health care providers are mandated to notify Public Health of reportable diseases as defined by the Minnesota Department of Health.

- Collaborative effort to address potential county-wide natural or manmade disasters and disease outbreaks

Public nuisance

- Community Health Nurses perform general public nuisance assessments and refer to appropriate governing agency

Nutrition Education

- Nurses provide nutrition education to various groups and individuals throughout the community to promote healthy nutrition and prevention of obesity and related conditions

NAPS Program (Nutrition Assistance Program for Seniors)

- Community Health facilitates referral of eligible seniors for a monthly food distribution program
