Provides a range of public health functions that protect and promote the health of residents. Services include:

• Collaborative Wellness Initiatives: Works with community partners, schools, workplaces, and organizations to create policies and programs that support healthy lifestyles and prevent chronic diseases (e.g., tobacco-free living, active living).

• Community Health Assessment and Planning: Leads or contributes to regular assessments of community health needs and develops improvement plans to address priority issues in the service area.

• Contagious disease/life-threatening illness investigation

• Contracts with school districts to provide pre-school screenings, hearing and vision screenings, scoliosis screenings, school health services, and health education

• CPR Training and Certification are available for a fee

• Environmental Public Health Assessments: Partners with regional teams to identify and address environmental health hazards in homes and communities (e.g., healthy homes approaches).

• Investigation and follow-up regarding reports of public health nuisance in which the health of the public is at risk or in danger

Services provided:

• Investigates and treats Tuberculosis (TB)

• Provides control function of managing communicable disease outbreaks

• Schedules immunization clinics for child/adolescent/adult immunizations.

• Screens and supports refugees and immigrants

Vaccines provided under the Minnesota Vaccines for Children Program and the Uninsured /Underinsured Adult Vaccine program:

• Adults: Tetanus, pneumonia, HPV (Gardasil), MMR (measles, mumps, rubella), Hepatitis A, Hepatitis B, Influenza, chickenpox, and shingles vaccinations. Any adult aged 19 or older who is underinsured or uninsured can receive these immunizations.

?- Children: All required vaccines for day care, Headstart, and school (preschool - high school). Plus recommended vaccines including Hepatitis A, Human Papillomavirus (HPV), Rotavirus, and Influenza.

Provide disease investigations, post-exposure medications, and immunizations to prevent and reduce the spread of infectious diseases. Services offered include:

• Infectious disease investigations

• Immunization and flu clinics

• Mass dispensing clinics

• Nursing staff provides health assessment for infectious disease and status of immunization for refugees that have arrived in the county

• Perinatal Hepatitis B case management

• Tuberculosis case management

Provides the following services and information to prevent the spread of diseases:

• Assists and conducts infectious disease investigations

• Communicable disease prevention, education, and follow-up to control the spread of disease

• Coordinates with the Minnesota Department of Health on case investigation

• Information on various diseases and conditions

• Immunization records and information

Provides services to assist with disease prevention and control in the community, including:

• Disease reporting

• Routine immunizations for children

• Sharps disposal

• Educates the public, individuals, families, day care providers, schools, businesses, and other groups about preventing the spread of communicable diseases such as Influenza, Tuberculosis, HIV, and others

Case management

• Ongoing communication with client, providers to ensure appropriate service delivery, medical follow through and client advocacy.

Disease prevention and control

• Ongoing education, outreach and surveillance of communicable diseases under the direction of the Minnesota Department of Health.

Emergency preparedness/bioterrorism

• Collaborative effort to address potential county-wide natural or manmade disasters and disease outbreaks

Public nuisance

• Community Health Nurses perform general public nuisance assessments and refer to appropriate governing agency

Nutrition Education

• Nurses provide nutrition education to various groups and individuals throughout the community to promote healthy nutrition and prevention of obesity and related conditions

NAPS Program (Nutrition Assistance Program for Seniors)

• Community Health facilitates referral of eligible seniors for a monthly food distribution program

Health Promotion and Wellness Activities

• Numerous health topics and wellness activities can be provided to various groups and individuals throughout the county

• Conducts surveillance, investigates, monitors, and puts prevention interventions in place for communicable diseases.

• Educates the public, individuals, families, day care providers, schools, businesses, and other groups about preventing the spread of communicable diseases such as Pertussis, Influenza, West Nile Virus, Tuberculosis, Meningitis, and others.
