- Provides household stability counseling, case management, planning and longer-term support to address multiple needs and barriers. Also assists clients with finding employment, creating resumes, etc. - Limited emergency financial assistance for transportation to employment for residents of Deephaven, Excelsior, Greenwood, Hopkins, Minnetonka, Shorewood and Woodland. This financial assistance is provided on a limited, case-by-case basis to help with prescription co-pays, minor car repairs, bus cards, employment related expenses, etc.

Services include connections to: - Free Fentanyl Testing Strips and Naloxone kits available at all locations - Free training on how to use Fentanyl Testing Strips, Naloxone, and information about overdose prevention - Funding for emergencies and transportation. Funding is limited. - Insurance support and funding to help cover costs of Suboxone or other medication assisted treatment - Other supports throughout Rice County and the surrounding area - Peer recovery support - Referrals to free chemical health assessments - Treatment and recovery resources, including Medication-Assisted Treatment (MAT)

NEMT provides Medical Assistance (MA) members with a safe, appropriate, and cost-effective mode of transportation to get to and from non-emergency medical service appointments. Members can arrange NEMT for one-way or round trips. Medical necessity determines the following types or levels of transport and ancillary services the member is certified to receive for transport to and from covered medical services. Information, resources, and referral office to help people through difficult times. Staff provide an immediate assessment of emergency needs and offer resources to help deal with homelessness, chemical dependency, domestic abuse, unemployment, gambling addictions, mental health problems, and help with public assistance. Additional services offered at the center include: - 7 bus passes for the general public daily; usually gone by 9:10 am. Can be accessed twice per month and an intake form must be filled out. - Free telephone line, mailboxes for people who are homeless, hygiene items, and household goods - Safe Bay is located in the parking lot and offers individuals and families living in cars and vans a place to rest without fear of crime, harassment, or being moved along by law enforcement or private security guards. Individuals and families can access showers, breakfast and housing navigation services provided by Daminao Center. - Work related transportation: - Gas vouchers: MUST have valid driver's license, proof of insurance, and proof of employment. Can only be accessed once per year. - Bus pass: MUST have ID and proof of employment. The value of a bus pass is $25 and can only be accessed once per year.

Pathway of Hope: Designated case management for families with children. Food Shelf: Available once per calendar month. Walk-in during open hours. Culturally-specific food is often available. Hygiene: Hygiene kits are typically available. Showers are available for people who are unhoused during specific hours. Transportation: Must be able to verify that the client has a permanent job for which they have not received their first paycheck or a medical appointment. Clients should bring a letter (on company letterhead) stating when they started working and when they will get their first paycheck. Transportation is limited to once a year. Case-by-case exceptions may be made depending on the transportation need. HeatShare: Serves NE and SE Minneapolis. Employment services: job leads, resume assistance, job search techniques instruction Seasonal services: Family Camp, coats for kids, holiday programs for Easter and Thanksgiving, and emergency disaster services. Referrals to rental assistance (funding is limited), car repair services, and budget counseling.

Food shelf which can be used once in a 30-day period. Families receive food for three - five days. Hygiene items may be available as supplies last. Vouchers for clothing (adults and families), and bedding/linens. Provides Bridging referrals for individuals needing furniture. Transportation: Bus and gas cards are available for individuals who are experiencing homelessness and in a Coordinated Entry program or for individuals who are starting a job but haven't received their first paycheck yet. HeatShare, seasonal services, Family Camp, back to school program, seasonal programs Coordinated Entry assessments for adult families and singles 24 and older in Anoka County Family Homeless Prevention and Assistance Program (FHPAP): Funds are only available to individuals and families facing an eviction. Funding varies month to month and may not be available. NOTE: One phone line is set aside for this program. It stays open until there are more than 80 requests per week. Then it is closes until staff are caught up on intakes. When the intakes are caught up, the line is opened again. Individuals should try the phone number a few times throughout the week. If someone asks, let them know to keep checking Line 5 - it will open again soon. Rapid Rehousing: Provides services for people experiencing homelessness that have found housing. Street Outreach: Provides outreach services such as food and transportation for people experiencing homelessness.
