Brooklyn Park Social Service Office - Salvation Army Twin Cities Social Services

Vouchers for clothing (single adults only), bedding and kitchen items. Household: Furniture is not free. Vouchers are given to assist clients in the purchase of furniture at a discount price at the Salvation Army Thrift Stores. Clients must verify they have recently left a shelter, or that they have a new living situation or other crisis situation through a case worker, pastor, school social worker, landlord, etc. Vouchers are capped at $100 per family and can not be used for beds. Transportation: Clients can qualify for tokens or a bus card in one of two ways. The caseworker must be able to verify by telephone that the client has a permanent job for which they have not received their first paycheck or a permanent job interview or has a medical appointment. Hygiene: Items such as shampoo, soap and toothpaste are available. Clients are eligible once every two months. HeatShare, seasonal services, Family Camp, free yearly Phone Home project, back to school project, and emergency disaster services. Food support application assistance (SNAP)

Services included: - Bus cards to get around - Case management - Clothing items - Financial counseling classes include bank accounts, budgeting, credit counseling, and savings - Food Resources - Hygiene products - One on one individual mentoring and goal planning - School needs such as school supplies and advocating on for children at school - Shoes - Young teen pregnancy (resources to help adolescents who dropped out of school go back and get their High School diploma or GED)

Social Services - ResourceWest

Provides household stability counseling, case management, planning and longer-term support to address multiple needs and barriers. Also assists clients with finding employment, creating resumes, etc. Offers limited emergency financial assistance for transportation to employment for residents of Deephaven, Excelsior, Greenwood, Hopkins, Minnetonka, Shorewood and Woodland. This financial assistance is provided on a limited, case-by-case basis to help with prescription co-pays, minor car repairs, bus cards, employment related expenses, etc.

A private, non-profit organization serving Clearwater County. (Requests from outside of the county will not necessarily be denied) The purpose of Salvation Army is to provide moral and spiritual support, material goods, food, shelter, and transportation for those unable to provide for themselves. Every situation is reviewed individually Conditions of service: - Will maintain anonymity and confidentiality - Will require name, address and Social Security number in order to provide ANY services. - If a person doesn't have a Social Security card, a driver's license may be acceptable. Emergency Housing Services - Tri-Valley Opportunity Council

Provides short-term help for low-income households who are homeless or at imminent risk of homelessness. Direct financial assistance may include foreclosure payments, rent payments, rent subsidies, security deposits, first month's rent, utility payments, or other assistance to prevent eviction. Minimal assistance is also available for food, child care, transportation, and work clothing and supplies. The purpose of these programs is to stabilize families in their home or re-house families that are homeless.

- Offers grants to help families with past due rent and mortgage, utility bills, short-term transportation assistance, school or job-related expenses, and prescriptions - Emergency supplies, laundry tokens, and clothing are available for people experiencing homelessness - Financial assistance follows county guidelines for Emergency Cash Assistance and must resolve the crisis or situation. Individuals can receive financial assistance once within a 12-month period.

Community Services - Damiano Center

Information, resources, and referral office to help people through difficult times. Staff provide an immediate assessment of emergency needs and offer resources to help deal with homelessness, chemical dependency, domestic abuse, unemployment, gambling addictions, mental health problems, and help with public assistance. Additional services offered at the center include: - 7 bus passes for the general public daily; usually gone by 9:10 am. Can be accessed twice per month and an intake form must be filled out. - Free telephone line, mailboxes for people who are homeless, hygiene items, and household goods - Safe Bay is located in the parking lot and offers individuals and families living in cars and vans a place to rest without fear of crime, harassment, or being moved along by law enforcement or private security guards. Individuals and families can access showers, breakfast and housing navigation services provided by Daminao Center. - Work related transportation: - Gas vouchers: MUST have valid driver's license, proof of insurance, and proof of employment. Can only be accessed once per year. - Bus pass: MUST have ID and proof of employment. The value of a bus pass is $25 and can only be accessed once per year.

Patient Travel Assistance - Blood Cancer United - Upper Plains Region

The program is available to blood cancer patients with significant financial need. Those who qualify will receive $500, once within a 6 month period, to cover the cost of treatment-related travel and lodging expenses for the patient and caregiver, including: ground transportation; tolls; gas; parking; car rental; car maintenance, repairs, and parts; air transportation; baggage fees; lodging; and ambulance services. The following expenses are not covered: food and beverages, alcohol, tobacco, clothing, medical/pharmacy expenses, and international travel. Any expense not covered by the program may be subject to audit and may result in loss of award.

Transportation - Reach for Resources

Offers a variety of safe, accessible, and reliable transportation for individuals living with disabilities who live in Minnesota. Transportation programs include: - Reach in Motion: Enables individuals to use county waiver or county funds to cover Lyft rides through a Lyft Pass arranged by Reach's Transportation Manager. Lyft rides do not have geographic limitations and out-of-pocket costs. - Trail by Reach: Provides transportation for adults with developmental disabilities living in Bloomington, Eden Prairie, Edina, Golden Valley, Saint Louis Park, and Richfield. Transportation rides are for recreation, leisure, and educational programs, if a program is hosted in one of the designated cities.

Winona Volunteer Services offers vouchers to meet transportation needs for people who are struggling. Before a voucher is given an in-depth conversation will take place between the individual and staff to determine individual's circumstances. Vouchers may be issued for: - Bicycles for individuals without transportation who are either looking for work through an employment agency or are currently employed and agree to forego all future requests for bus passes, gas, etc - Bus passes (city) for work, school, or seeking employment - Bus or train tickets for individuals or families needing a one-way ticket in verified emergency situations (for after-hours transportation emergencies contact Law Enforcement Dispatch Center) - Gas for valid and insured driver working with an employment agency to job search or experiencing unexpected circumstance such as reduced work hours - Taxi cabs vouchers may be used in urgent circumstances

NEMT provides Medical Assistance (MA) members with a safe, appropriate, and cost-effective mode of transportation to get to and from non-emergency medical service appointments. Members can arrange NEMT for one-way or round trips. Medical necessity determines the following types or levels of transport and ancillary services the member is certified to receive for transport to and from covered medical services.
