Care consultation provides adults age 60 or older, their caregivers, and families with knowledge, skills, tools, and resources to help older adults lives as independently as possible. Consultants work over the phone or in the home to identify services to meet needs. There are no income guidelines, and services can be provided for as long as needed, with no minimum or maximum visits required. Care Consultants can: - Advocate on your behalf to ensure you receive the services you need - Connect clients to services that increase health and independence - Create customized action plans - Educate regarding chronic disease management and prevention - Provide family mediation - Fill out legal documents such as health care directives - Identify community resources - Identify risks in the home that compromise health and safety - Help with managing difficult family decisions - Help with planning for the future - Help with problem solving

Provides lead education, outreach and blood lead screening. Services include: - Baby Safe at Home: Provides free home visits to identify lead hazards in the home; qualifying households will be provided with safety items that protect babies from household hazards - Community outreach, presentations on preventing childhood lead poisoning for both children and adult audiences - Healthy Homes: Help identify and take care of safety and health hazards in the home - Screening for lead in the blood, and in-home education visits. During the in-home visit SRC staff will test the home for lead hazards.

Helps saves lives in communities by installing smoke alarms in homes and educating people about home fire safety. During a home visit, residents receive: - Emergency preparedness tips and information - Guidance in the development of a Family Disaster Plan - Up to 3 installed smoke alarms if no working smoke alarms are present in the home Demonstration and installation services are performed by Red Cross-trained volunteers and/or community partners or local fire departments.
- Provides education and car seat distribution to eligible clients - Specific car seat availability and eligibility depend on insurance, child age, and resource availability as determined by intake. Car seat distribution appointments take 60 - 90 minutes - Provides car seat checks for those with existing car seats that want to make sure the seat is installed correctly
Physical therapy uses exercises, manual therapy, and other techniques to treat conditions that affect an individual's physical function. Physical therapy may help an individual: - Avoid the need for medication or surgery - Improve mobility, coordination, flexibility, and strength - Recover after an injury, illness, or surgery - Reduce pain - Regain independence Therapists may also conduct evaluations like functional capacity evaluations. Fall Prevention Physical therapists work with individuals to prevent falls by: - Assessing an individual's risk factors, including certain medical conditions and medications - Educating an individual about fall prevention - Helping make an individual's home safer - Providing dizziness and balance care - Teaching an individual exercises to improve their strength and stability - Testing an individual's balance and ability to walk Orthopedic Rehabilitation Physical Therapy Type of physical therapy that's used to treat conditions like: - Arthritis - Back, neck, jaw, joint, or muscle pain - Broken bones - Injuries or conditions that affect the shoulder, elbow, hip, knee, foot, or ankle - Sprains and strains - Sports and exercise injuries Physical Therapy for Children Therapists help children improve functional gross motor skills like rolling, sitting, walking, or skipping. Physical therapists also teach children how to use a wheelchair, walker or other type of mobility equipment. Specialized PT Services Physical therapists offer specialized physical therapy services for the treatment of: - Back and neck pain - Balance and dizziness-related problems - Hand conditions - Lymphedema - Neurological conditions like stroke or brain injury - Pain rehabilitation - Pelvic floor disorders

Offers certified Child Passenger Safety Technicians to assist caregivers with car seat installation and education on Minnesota's Child Passenger Restraint Law. Technicians provide guidance on proper use and fit for rear-facing, forward-facing, and booster seats, as well as seat belts. Free car seats are also available for qualifying low-income families or those on Medical Assistance.
La Crescent City Police Department provides law enforcement services for the community. Services include patrols, investigations, arrests, fingerprinting and background checks (including pre-employment), animal control (domestic and wild animals), parking control, weather sirens, noise violations, curfew ordinance enforcement, bicycle safety/registration, and crime reporting and prevention programs.

- Blood pressure screening - Breastfeeding mothers support - Case management and care coordination - Child and teen checkup outreach - Family planning services - Financial assistance available for prenatal care, pelvic exams, PAP smears, pregnancy testing, etc. - Follow Along Program: Home visits for newborns, including questionnaires sent to the home until child is four years old. If something on a questionnaire causes concern, a nurse will visit the home and take action if appropriate. - Food recall information - Home health care including home nursing with blood pressure screening, diabetes education for newly diagnosed patients, instructions for new diets, and monitoring of medication administration. Home Health Aides assist with bathing and personal care, meal preparation, and assistance with hair and oral care. Physical therapists, occupational therapists, and speech therapists provide services also. - Immunizations: available for children and young adults, birth through age 18. Hepatitis B and Tdap for a fee for adults. Flu shots available from October through December. - Mantoux testing (tuberculosis) - Loan closet - Maternal and child health (car seats available for low-income families) - Prenatal care and education including safety and proper use of car seats - School health nurses consult with teachers on topics such as AIDS, teen pregnancy prevention, self-breast exams, and nutrition. Preschool screenings as well as immunization, vision, and hearing clinics.
