Provides a range of primary care options including family medicine, geriatrics, medical home, pediatrics, sexual health, and virtual visits.Services include: - Diagnosing and treating illnesses, infections, and injuries- Family planning/birth control- Growth and development monitoring- Health education on nutrition and exercise- Male and female specific health concerns- Managing chronic conditions- Preventative care visits- Preventive care, such as well-child checkups and routine screenings- Screening tests- Vaccinations

Primary Care Clinic

Provides or arranges support for elder neighbors to assist them in living at home as long as possible. This support, which may otherwise not be available, uses the program's small staff, volunteers and/or paid nursing staff. Services include:- Caregiver coaching and support groups- Community Gardening- Door-to-door transportation, both assisted and non-assisted, provided by volunteers. There is no charge, but donations are accepted. Accessible vehicles can be requested.- Elders resource list of meal and grocery delivery, handyman services, modifications, etc. available by phone or Internet- Exercise classes- Friendly visiting- Health education classes and events- In-home eldercare assessment and resource recommendation- In-home Internet/computer tutorial- Linkage to county services- Nursing referrals- Phone visiting- Respite care- Seasonal raking and snow shoveling- Shopping assistance up to twice a month- Spring/Fall yard clean-ups- Volunteer opportunities

Health education programs offered to create within individuals & the community, an understanding and a desire to take part in solutions to their own health problems Blood Pressure Screenings. Trained volunteers provide blood pressure screenings in communities throughout Cass County. Public health nurses are available for follow-up and educational services Cholesterol Screenings.
