Home and Community Care: Provides and coordinates many services through a continuum of care for Veterans in their homes and community settings, including Home-Based Primary Care, Medical Foster Home Program, and providing programs and services that are delivered by community partners.

Physical Medicine and Rehabilitation: Provides a full array of acute inpatient and outpatient rehabilitation services for Veterans with a variety of disorders, primarily neurological and musculoskeletal. Specialized CARF accredited rehabilitation programs such as Amputee, Traumatic Brain Injury/Polytrauma, Stroke, Assistive Technology, and Chronic Pain. (612)629-7797, 8 am - 4:30 pm Monday - Friday.
Blind/Vision Rehabilitation Services: Provides a continuum of care for severely visually impaired, TBI, low vision, and blinded Veterans and service members. Services can be delivered in the home, a VA facility, community setting, or at a Blind Rehabilitation Center, and include: adjustment to blindness counseling, patient and family education, support groups, benefits analysis, short term and comprehensive residential training (VISOR Program and HOPTEL and Non-HOPTEL Program), outpatient rehabilitation services, and the provision of prosthetic aids. Post 9/11 Transition and Care Management: Provides reintegration/transition assistance, care/case management, and outreach services for Veterans who have served in combat since November 11, 1998, and active duty service members. 4th Floor, Clinic 4K, (612)467-3757 or (866)414-5058 Ext. 3757, 8 am - 4 pm Monday - Friday.
Physical Medicine and Rehabilitation: Provides a full array of acute inpatient and outpatient rehabilitation services for Veterans with a variety of disorders, primarily neurological and musculoskeletal. Specialized CARF accredited rehabilitation programs such as Amputee, Traumatic Brain Injury/Polytrauma, Stroke, Assistive Technology, and Chronic Pain. (612)629-7797, 8 am - 4:30 pm Monday - Friday.
Blind/Vision Rehabilitation Services: Provides a continuum of care for severely visually impaired, TBI, low vision, and blinded Veterans and service members. Services can be delivered in the home, a VA facility, community setting, or at a Blind Rehabilitation Center, and include: adjustment to blindness counseling, patient and family education, support groups, benefits analysis, short term and comprehensive residential training (VISOR Program and HOPTEL and Non-HOPTEL Program), outpatient rehabilitation services, and the provision of prosthetic aids.
