Alzheimer's and Dementia Care: Provides individualized assessments for evaluating cognitive and functional levels. Services include:
- Promoting independence in dressing, bathing, toileting and assistance with memory, communication and swallowing difficulties
- Provide education with addressing physical health needs, medication management, strength development and fall risk reduction
- Teaching families and caregivers strategies for managing the care needs of their loved ones
- Teaching how to effectively respond to challenging behaviors such as aggression, agitation, and repetition
- Treatment planning

Alzheimer's and Dementia Care: Provides individualized assessments for evaluating cognitive and functional levels. Services include:
- Promoting independence in dressing, bathing, toileting and assistance with memory, communication and swallowing difficulties
- Provide education with addressing physical health needs, medication management, strength development and fall risk reduction
- Teaching families and caregivers strategies for managing the care needs of their loved ones
- Teaching how to effectively respond to challenging behaviors such as aggression, agitation, and repetition
- Treatment planning
Home Health Aide (HHA): Home Health Aides are responsible for documenting the care provided to individuals, and notifying the case manager of any changes in health. Home Health Aides may assist in the following areas:
- Assistance with dressing
- Bathing, washing hair and shaving
- Getting in/out of bed
- Maintaining a safe living environment
- Monitoring blood pressure, temperature and other vital signs
Skilled Nursing: Provide in home care services while recovering from an illness, injury, or event. Services may include:
- Bowel and bladder care
- Infusion Therapy
- Medication setup and dispensing
- Ostomy care
- Pain management
- Wound care/dressing changes
Skilled Therapies: Provides in home occupational, physical, or speech therapy to assist with the recovery process
