Consultation Services for Community First Services and Supports (CFSS) - Arc Northland

Community First Services and Supports (CFSS) is a Minnesota health care program that offers flexible options to meet the unique needs of people. CFSS allows people greater independence in their homes and communities. CFSS replaces personal care assistance (PCA) and the Consumer Support Grant (CSG). CFSS covers the same main services as PCA: - Activities of daily living, such as eating, bathing, grooming and transferring - Health-related tasks - Instrumental activities of daily living, such as shopping, cooking, laundry, and assistance with self-administered medications - Observation and redirection of behavior. Under CFSS, individuals will work with a Consultation Services Provider, like Arc Northland, to be able to choose between the Budget Model or Agency Model. Consultation Services Providers will provide education on their options and provide support in writing their service delivery plan.

Senior Programs - Keystone Community Services

Active Seniors: Provides adults 55+ with a variety of diverse activities such as exercise, wellness, and social/recreation designed to support independence Care Consultation: Keystone Care Consultants help seniors navigate their health, economic, and social needs. Care managers meet with individuals in their homes or at one of the community centers to conduct an assessment, gather input from caregivers, and create goals. Once the assessment is complete, services may include: setting up rides to a social program, researching housing options, making a referral to an agency that helps with grocery shopping and/or cleaning, providing caregiver support, and accompanying seniors to doctor visits. Grocery Delivery: Provides free grocery delivery for low-income seniors and adults with disabilities. Those who qualify can receive monthly deliveries. Delivery can include meat, dairy, fresh produce, and shelf-stable items. In each delivery, individuals will receive an average of 25 pounds of food per person in the household. Accommodations are based on customer choice models such as vegetarian, halal, etc. depending on what is available. Deliveries will be dropped off between 12:15 pm and 1:30 pm Tuesday - Thursday by a volunteer. In-Home Help: Social workers coordinate with Recover Health to provide seniors with in-home nursing care, in-home therapies, and home health aides to assist with daily care. ?Tax Preparation Assistance: Provides tax services in coordination with AARP to seniors. Home Health Care Services - AbbeyCare

Companion/Night Supervision Services: Services that help an individual work toward a therapeutic or community integration goal. Night supervision is an overnight assistance and monitoring provided in the person's own home, which includes assistance with Activities of Daily Living (ADL's), reinforcing independent living skills, etc. Homemaking/Home Management Services: Provide assistance to individuals who are unable to perform day-to-day household duties and have no one available to assist them. Services include light housekeeping, laundry, limited personal care, grocery shopping, meal preparation, etc. Individual Community Living Support (ICLS): Provides assistance and support to clients who need reminders, cues, intermittent/moderate supervision, or physical assistance to remain in their own home. Services include assistance with active cognitive support, Activities of Daily Living (ADLs), household management, health, safety, and wellness, and community engagement. Personal Care Assistant (PCA): Performs care services to clients unable to live independently in the community without assistance. Services include assistance with Activities of Daily Living (ADLs), grooming/personal hygiene, light cleaning, cooking, etc. They also engage clients in activities such as reading, talking, community support, etc. Personal Support Services/Individualized Home Supports Without Training: Provided in the individual's home to achieve their full potential, to increase independence, and to meet community inclusion goals that are important and based on their need. Established to assist with Activities of Daily Living (ADLs), participation in community activities, etc. Respite Care: Short-term care services provided due to the absence or need for relief of the family member(s) or primary caregiver normally providing care. Respite can be provided in-home or out-of-home.

Individuals fill out a questionnaire. Answers are reviewed by an Essentia provider who will make a diagnosis and create a care plan. Prescriptions may be provided. Individuals are also given a phone number to call if they have questions or concerns. The process usually takes 60 minutes or less. Conditions Treated Allergy symptoms, like sneezing, runny nose, or eye irritation Bug bites, including tick bites Colds Eye infections, such as conjunctivitis (pink eye) Irritable bowel syndrome Low back pain Mouth sores, including canker sores - Skin conditions, including Acne Dermatitis Diaper rash Eczema Mild burns Shingles Tinea infections, also called ringworm Tobacco use Upper respiration infections, including sinus infections or influenza Urinary tract infections (UTIs) in women Vaginal yeast infections Stye COVID-19 screening is also provided.
