Caregivers are licensed nurses, social workers, therapists, and certified aides. Home-based services include medical and non-medical options such as: - Home Health: Focuses on medical assistance including medication management, rehabilitation services, wound care, social work, and other medical treatments. Other specialized services include postsurgical rehabilitation and evaluation, physical speech, occupational therapies, disease or symptom management, catheter care, and IV therapy. - Home Care: Assists with daily living activities, light housekeeping, meal preparation, companionship, medication reminders, and more - Telehealth Services: In-home technology to let caregivers communicate with patients remotely. Telehealth devices include blood pressure monitors, pulse oximeters, medication management systems, and video calls with a care provider. - Hospice: Hospice care can be provided in the home, at an extended care facility, or inpatient hospice care facility. Provides quality of life to patients and caregivers who have a life-limiting illness. A team of professionals collaborates with the patient and their family in the decision-making process to manage symptoms so their last days may be spent with dignity.

Home Care Specialized nursing care is provided by public health nurses in the home to assist with medication management, wound care, dressing changes, post-operative care, and other services ordered by the client's physician. The public health nurse may also assign a licensed practical nurse or home health aide to assist in the care of the client. Home health aides/elder care aides provide personal care (hair washing, showers, nail care, etc.), visiting, light housekeeping, and other services for clients with medical or health problems. Aides may also monitor vital signs and provide basic health services. The public health staff is available to perform home safety checks for families with children or for elders. A limited number of safety items may be available following the home safety check. Instruction on how to use this equipment will be provided, although the nursing staff cannot install the equipment for families. Community Health Representatives (CHR) Transportation Provides transportation for individuals to medical, dental, and optical appointments only after all other resources are exhausted. Appointments should be confirmed with the CHR's schedule to assure that clients arrive on time to their appointments. No one under the age of 18 will be transported without a parent. All persons being transported are required to wear a seat belt at all times. Delivers medical prescriptions to homebound elders who have no other means of obtaining their medication.

Provides home health care services to help individuals who want to remain living independently in their home. The goal is to help individuals avoid rehospitalizations and emergency room visits. Services may include: - Home Health Aides: Provide periodic personal care, light meal preparation, housekeeping, and give verbal or visual reminders for performing scheduled treatments and exercises - Nurses: Monitor patient health for managing illness, infection control, wound care, home safety, medication management, and IV therapy - Therapists: Offer physical, occupational, and speech therapy to improve and restore skills that were lost or diminished due to illness or accident.
Individuals fill out a questionnaire. Answers are reviewed by an Essentia provider who will make a diagnosis and create a care plan. Prescriptions may be provided. Individuals are also given a phone number to call if they have questions or concerns. The process usually takes 60 minutes or less. The Living At Home Block Nurse Program provides or arranges whatever support an elder neighbor needs to continue living at home as long as possible. This support, which may otherwise not be available, uses the program's small staff, volunteers and/or paid nursing staff.
