In-Home Support Program

For individuals who have a disability and still live in their family's home or in foster care. Individuals receiving services have 1:1 staffing. Flexible based on individual needs. Services are provided in the family home, at a Hiawatha location or elsewhere in the community. Activities are customized to the individual's wants and needs. Examples of activities are similar to those of the LEEP program, but may also include:

- Assistance in getting up and ready for school/work

- Assistance with homework

- Participation in evening or weekend leisure activities

Services include: - Faith Residence Apartments: Offers independent living for individuals 18-62 years living with disabilities in 1-2 bedroom apartments; tenants must meet the Department of Housing and Urban Development (HUD) income requirements. - Hope Residence: Offers services and support using person-centered care, which includes nursing onsite 16 hours a day, assistance with daily activities, and community activities.

- Budgeting, paying bills, and gaining financial skills

- Community participation, including accessing transportation, building networks, and personal safety

- Independently taking medications

- Meal planning and preparation, including nutritional education

- Obtaining and maintaining a home

- Personal appearance and hygiene

- Shopping

• Accessible, affordable housing and supportive living services for adults with a traumatic brain injury and/or mobility impairment.

• Residents receive case management support, personal care services, independent living skills training and assistance with cooking and cleaning on a 24 hour basis.

• Each resident has a large, private bedroom and access to spacious common areas. Call system to alert professional of resident's needs.

Operates adult foster care homes throughout central Minnesota. Provides safe, comfortable living environments for individuals with developmental disabilities or mental health needs. Each home offers a family-type setting where clients receive personalized care tailored to their specific needs. 

Also offers a range of community-based services, including Semi-Independent Living Services (SILS), Independent Living Services (ILS), Supervised Living Services (SLS), respite care, in-home family support, and support for college students through the Occupational Support Service Program (OSP). These services are designed to promote independence and community integration for individuals with special needs.
