Respite - Short-term care services provided to a person when their primary caregiver is absent or needs relief. Services include:

• In-home respite: Service is provided to a person in their home or the family home.
• Out-of-home respite: Service is provided to a person in a licensed or unlicensed setting that is not the person's home.
• Respite level A: Active, out in the community
• Respite level B: Stay at home and local outings only
• Respite level C: Sleep in
• Respite level D: 24-hour period

Homemaker Services - Services that help a person manage general cleaning and household activities. Services include:

• Cleaning: light housekeeping tasks, laundry services
• Home management: cleaning, transportation arrangement, meal preparation, shopping for food, and household items
• Assistance with ADLs: cleaning, assistance with ADLs (ambulating, bathing, dressing, eating, grooming, toileting)

*Training is not covered under this service. To receive homemaker services, homemaker cleaning must be the primary service provided. County-Funded Semi-Independent Living Services (SILS) - Services needed to maintain and improve an adult's capacity to live in the community. Supports people in ways that enable them to achieve personally desired outcomes and lead self-directed lives including:

• Activities of Daily Living (ADLs): Activities a person needs to carry out on a daily basis to remain healthy and safe including dressing, grooming, bathing, eating, positioning, transfers, and mobility.

• Instrumental Activities of Daily Living (IADLs): Activities a person needs to carry out on a regular basis to remain independent including shopping, paying bills, and meal preparation.

• Health-related procedures and tasks: Tasks such as supporting a person with self-administered medications or help with range of motion exercises.

• Observation and redirection of behaviors: Monitoring a person's behaviors and redirecting them to more positive behaviors when needed.

• Residential programs designed to provide individuals with developmental disabilities, the services and supports they need to live in the community.

• The in-home family support for adults and children is available.

Residential and support services for persons who are developmentally disabled or who have conditions similar to a developmental disability, including traumatic brain injury.

• Faith Residence Apartments: Offers independent living for individuals 18-62 years living with disabilities in 1-2 bedroom apartments; tenants must meet the Department of Housing and Urban Development (HUD) income requirements.

• Hope Residence: Offers services and support using person-centered care, which includes nursing onsite 16 hours a day, assistance with daily activities, and community activities.

Provides supported living for persons with developmental disabilities. Solid Foundations has a contract with Itasca County Human Services to provide this housing. Clients must go through ICHHS case worker to determine eligibility.

Corporate Foster Care: Provides corporate foster care to children and adults with serious and persistent mental illness, and individuals with traumatic brain injuries. Programs are based in residential neighborhoods, usually with four individuals of similar needs and interests living together. Provides a living environment that promotes as much independence as possible, within a supervised setting. Training is provided in a range of areas, including environmental and behavioral strategies, daily living activities, and independent living skills. Supported Living Services: Provides supported living services to children and adults with developmental disabilities. Programs are based in residential neighborhoods, usually with four individuals of similar needs and interests living together. Provides a supported living environment that promotes as much independence as possible, within a supervised setting. Training is provided in a range of areas, including environmental and behavioral strategies, plus daily living activities such as social skills, community integration, and recreational and leisure activities. Provides independent living skills instruction such as money management, domestic tasks, transportation training, personal hygiene, program solving skills, among others.
