Community-Based Services and Supports - Dungarvin Minnesota

- 24-Hour Responder Service: On-call staff help with health and personal injuries. The service is often paired with Independent Home Support with Training program. Staff will respond by phone or in person depending on the level of services. - Acute Behavioral Services for Children and Adults: Services provided to individuals with interfering behaviors and/or anti-social issues, who have a history of law enforcement involvement, have difficulty living with others and require high staffing needs, are in frequent crisis situations, or coming out of an institution. - Case Management: Available to residents of Hennepin, Ramsey, and Wright Counties who receive funding through various waivers and/or programs. Assists navigating the social service system and provides information and advocacy. - Integrated Community Supports: Provides support and training/habilitation in community living service categories to adults ages 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person's living unit or in the community. - Individualized Home Supports: Provides a blend of in-person and remote services that focus on support and training of a person who needs to maintain or increase independence for their continued experience living in the community. Also offers services to relatives working to support the person's goals. - Residential Services: Operates residences for adults with significant behavioral, medical, or physical challenges including brain injuries. Residential group home programs are licensed as adult or child foster care, or a supervised living facility. Each site serves 2 - 6 people. - Supportive Employment Services: Provides individuals with intellectual and developmental disabilities assistance with initial stages of the employment assessment period, job search and development, job coaching, and working independently - Vocational Rehabilitation Services: Offers a variety of services including job-seeking skills training, job coaching, social coaching, post-secondary education, and more. Basic and In Home Support Services - Affinity Caregiver

Provides basic and in home support services for older adults and people with disabilities in their homes or in a community setting to ensure the health and safety of the person and maintain their independence within the community. Basic Support Services - Individual community living supports (ILCS): For people who need reminders, cues intermittent/moderate supervision, or physical assistance to remain in their own homes. - 24-hour emergency assistance: On-call counseling and problem-solving and/or immediate response for assistance at a person's home due to a health/personal emergency. - Companion services: Non-medical care, supervision and socialization to a person age 18 or older. - Night supervision: Provides overnight assistance and monitoring by an awake staff in the person's home. - Respite care services: Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care. Intensive Support Services - Positive support services: Services to increase positive behavior and decrease or eliminate severe, challenging behavior. - Specialist services: Services designed to promote staff and caregiver competency to meet a person's needs in eligible areas. - Independent living skills training: Services that develop, maintain and improve the community-living skills of a person. - Semi-independent living skills: Services needed by an adult with a developmental disability or related condition(s) to live successfully in the community. - Residential-based habilitation: Including in-home family support and supported living services for adults in their own home: Supported living services provided in a licensed site, including a foster care home, a community residential setting, or a supervised living facility.

Behavioral and Residential Services - Saint Francis Health Services - Minnesota

Provides homes and services to people with developmental and delayed disabilities, mental illness, traumatic brain injuries, and other health-related needs wherever identified in communities throughout Minnesota. Individuals can participate in day placement, day treatment, and vocational readiness on a regular basis. Adult foster care - Provides food, lodging, and supervision services to a person with disabilities, mental illness, and brain injury. These services can be short term or long term. 62 residential program sites/ 261 beds. 12 sites offer a 5th bed or apartment with services, for 25 total beds. 4 AFC beds. Children's foster care - A 24-hour substitute care for children placed away from their parents or guardians and for whom the state agency has placement and care responsibility. 4 residential program sites/8 beds. Community-based services - Community support programs - Independent living services - In-home services, including training for of the person and family members to increase their capabilities to care for and maintain the person in their home - Personal care assistant - Respite care services - Supported employment - Supported living services Intensive residential treatment - Short term for up to 90 days. 10 bed facilities at Brentwood (Marshall) and Fresh Start (Duluth). Brentwood has 1 of the 10 beds as a swing bed (crisis and IRT bed). Fresh Start serves an all-female population. 20 total IRT beds. Intellectual disabilities - Provides an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID). Individual must have a developmental disability or related condition by their 22nd birthday and requires a 24-hour plan of care. 10 facilities, 58 beds. 5 beds at Arlington in Fergus Falls and Rollingwood in North Mankato. 6 beds at Prairiewood, Rosewood, and Scenicwood in Alexandria; Lakewood in Spicer; Northwood in Red Lake Falls; Eastwood and Westwood in Redwood Falls; and Ravenwood in Morris.
