Creates sustainable, culturally-appropriate approaches to and tools for preventive community health to support the Somali community in the Twin Cities. Community Health Workers work with the community to promote behavior changes that will reduce the risk of diabetes, heart disease and stroke by providing culturally appropriate and specific programs, messages and interventions. Activities include a culturally-specific pre-diabetes and weight management informational DVD in Somali to supplement the curriculum, exercise classes for elders and grocery store tours to promote healthy food choices. Provides training to physicians and Community Health Workers that improve care and reduce health disparities.

Somali Child Spacing Program: Provides culturally appropriate reproductive health education and resources to the Somali community and to health care providers serving the community. Community Health Workers assist Somali men and women to understand their reproductive health options and how to access resources and care. Services include: - Home visits and community dialogues conducted by Community Health Workers to discuss the benefits of child spacing and to refer participants to reproductive health services. - Development and distribution of reproductive health education materials for providers and community members. - In-service trainings and forums for health care providers, educators and other social service professionals. Karen Family Planning Program: Provides culturally appropriate reproductive health education and resources to the Karen community and to health care providers serving the community by: - Improving participants' confidence in accessing health care, from understanding what types of care are available to knowing how to schedule appointments. - Increasing participants' comfort engaging in conversations about reproductive health and family planning, especially with their health care providers and their children. Also partners with health care professionals and community members to create health education tools intended to facilitate dialogue between Karen patients and their providers around the subject of family planning methods.
