- Adult day services - Atmosphere that promotes social relationships, self-sufficiency, and opportunity for growth - Community-based group employment under supervision of trained staff; services include case management, training, and advocacy - Assessment: Measures a person's strengths, interests, and work skills - Work Adjustment Training: Develops job skills to assist individuals in career planning specific to individual goals - Job Coaching: One-to-one training on employment skills - Job Development: Develops employment options for community placement, assists with resume preparation, interviewing skills, provides employer services, and matches participant's desires to employer - Supported employment provides training and independent placement of individuals desiring jobs in community settings - Center-based employment provides supervised employment opportunities in light industrial packaging and manual assembly, shrink wrap, skin wrap, and blister packaging - Follow-up Services: Professional job retention support - Life skills classes and social activities - Day Training and Habilitation Program - DT&H - Traumatic Brain Injury Program - Vast majority of participants are transported to and from their homes on ProAct vehicles which include specialized equipment, wheelchair lift buses, commuter vans, and cars - Vocational skill development

Provides health services and education to improve the well-being of White Earth Nation band members. Services include: - Ambulance, EMT, and paramedic services - Child passenger safety seat program - Diabetes project providing education and nutrition information. Fitness centers are available for personal training, group training, and other health-related incentive programs. - Niin Gikenjige coalition to break social stigmas surrounding HIV and Hepatitis C by providing education, prevention programming, testing, and referrals to medical services where appropriate - S.O.S. program offers workshops and events about promoting a healthy and addiction-free lifestyle - SNAP nutrition education to make informed decisions on health and nutrition - Tobacco-free communities program educates on tobacco use and offers cessation support and resources

Maternal and Child Health Program - Mille Lacs County Community and Veterans Services - Human Services Programs

Helps ensure the health of pregnant, and postpartum women, infants, and special needs children. Information and assistance are provided through public health clinics or home visits such as: - After-home care and resources to assist new mothers such as exercise programs, health needs, parenting education, newborn care, nutritional needs, and infant growth and development - Car seat program providing education and distribution of car seats for children birth to 8 years - Child immunizations, physical development screening, and well-baby checks at public health clinics - Fetal Alcohol Syndrome/Fetal Alcohol Effect including home visits - Guidelines for parents to provide safe care and a healthy environment for their child - Identification of problems and early intervention activities in conjunction with families and schools to assist families with special needs children - Nutritional assessment and counseling/education - Prenatal health needs include nutrition, weight gain, exercise, and substance use - Preparation of labor and delivery - Prevention of child maltreatment - Special care for high risk pregnant women - Supportive community resources

Residences - Beacon Specialized Living - Minnesota

Provides residential care facilities and services such as: - Foster homes in licensed and unlicensed settings for adults and children living with disabilities. Ongoing residential care and supportive services are individualized based on the needs of each person. - Crisis services and respite care to children up to age 18 who are in a crisis situation and are unable to remain in their current placement. Family Health - Meeker County Public Health

- Home visits to pregnant women to instruct on healthy pregnancy behaviors and preparation for labor and delivery - Home visits to families with newborns and children; instruction on child growth, development, parenting, and child care - Education, support, and referrals for pregnant women with substance use disorder. A nurse, social worker, and a chemical dependency counselor work together to provide intensive home-based case management services to support healthy substance-free pregnancies and lifestyles.
