Provides information, resources, and support needed to move life forward. These partners help navigate the challenges of MS unique to each individual's situation. In addition to connections to other MS Society services navigators: Navigate through the complexities of finding healthcare providers, benefits, insurance and access to coverage, and employment Offer financial assistance with obtaining durable medical equipment and aids for daily living, emergency financial assistance for help with housing or utility shut off, respite care, chores, home modifications, and more. Will assist individuals with future planning. Help provide wellness strategies that can improve quality of life including suggestions on diet, exercise, emotional well-being, and connection to local resources Assess for personalized case management through the Edward M. Dowd Personal Advocate Program Provide crisis intervention in times of need

Homes are designed to keep the staff to resident ratio low to provide high levels of interaction and support. - Activities and recreational programming tailored to each individual's interests - Flat rate pricing includes on-site doctor's visits, transportation and attendant at other medical appointments, transportation to activities in the community, laundry, cooking, meals, snacks, cleaning. Can do 1- and 2-person transfers, foyer lifts, incontinence care, and most medical conditions. Hospice care can be added as an end of life service when needed.

Group Homes - ACR Homes

Operates group homes located in 58 sites throughout the five-county suburban Twin Cities: - 2 - 6 person homes in suburban locations for those with disabilities; typical ratio of two staff for four residents - ACR keeps residents actively engaged in the community with activities that are meaningful to clients - Each client typically has their own bedroom and the rest of the home is shared - Each home has a live-in supervisor which helps to keep knowledgeable communication with loved ones as a component of care - Most homes are wheelchair accessible, and have an accessible van. Will make and take individuals to doctor's appointments and other activities. - Supports clients with g/j tubes, ostomies, catheters, oxygen, nebulizers, vest treatments, individuals who require range-of-motion and other therapies such as standing frames, braces, and mechanical transfers.
