Community Health Workers work with the community to promote behavior changes that will reduce the risk of diabetes, heart disease and stroke by providing culturally appropriate and specific programs, messages and interventions. Activities include a culturally-specific pre-diabetes and weight management informational DVD in Somali to supplement the curriculum, exercise classes for elders and grocery store tours to promote healthy food choices. Provides training to physicians and Community Health Workers that improve care and reduce health disparities.

Provides state-funded food assistance to legal noncitizens ages 50 or older who do not qualify for the Supplemental Nutrition Assistance Program (SNAP, formerly called Food Stamps or Food Support) because of their citizenship status. Monthly benefits are loaded on an Electronic Benefit Transfer (EBT) card, which looks and works like a debit card. The card can be used to buy groceries at authorized stores, farmers markets, and online retailers. Learn more about where to shop and what can be bought. The amount of benefits received is based on income, expenses, and number of people in the household.

- Household Goods Distribution: Provides quarterly and seasonal distribution events for those in need including cleaning supplies, food boxes, shelf stable items, diapers, PPE, light bulbs, winter clothing. Available items vary and are first come, first serve. - Meals on Wheels: Provides Halal meals to seniors age 60 and older or individuals who have a CADI Waiver - Summer Food: During the summer, youth ages 18 and under can receive prepared meals, breakfast and lunch, daily. Must eat meal on-site at community locations or the agency main office.

Provides financial assistance for an individual to travel to care for or visit an immediate family member with a serious health condition that arises in emergency circumstances. The acute injury or illness must pose an immediate risk to a person's life or long-term health. Other instances where the fund may be accessed is a medically necessary surgery or 1 post-operation visit. A maximum of $400 in assistance is provided per patient per calendar year which includes: - $.21 per mile traveled - $70 per night lodging - $40 per day for meals
Programs geared toward helping Cambodian elders and other southeast Asians in the following areas: - Applying for public benefits - English instruction - Citizenship classes, INS interviews and immigration assistance - Healthy eating promotion which reconnects participants with the eating habits of their home countries through nutrition education, development of community gardens, and meals served three days a week from a local restaurant. Serves elders from the Cambodian, Chinese, and Karen communities. - Physical activities and active living - Cultural and traditional ceremonies - Health education - Medicare application - Housing assistance - Funeral assistance - Recreational activities - Group shopping and medical appointment transportation - Translation

Cash-grant program for families with low incomes experiencing household emergencies. Families can receive assistance once every 12 months. The assistance received must resolve the crisis. Families must use their own money first. The amount of assistance provided might not cover the entire emergency, but it can help. The types of assistance the program may help with are for rent assistance, rent deposit, electric, gas, heating fuel, water, burial assistance, or other emergency need based on available funding.
