The Living At Home Block Nurse Program provides or arranges whatever support an elder neighbor needs to continue living at home as long as possible. This support, which may otherwise not be available, uses the program's small staff, volunteers and/or paid nursing staff. Services include:
- Advocacy
- Blood pressure checks
- Caregiver and respite
- Case management
- Door-to-door transportation provided by volunteers. Vehicles are not wheelchair accessible.
- Flu shots
- Home health aides
- Information & referral, including to county services
- Phone visits
- Service coordination
- Skilled nursing care
- Volunteer errands and visits
- Volunteer services
