FACTS Family Healing - Nexus Family Healing
Outpatient/Community Mental Health Services
 
Provides licensed therapists who are specially trained in trauma and stress to support youth, families, and adults, ages birth to adult. Offers in-person, in-home, and telehealth options.This includes individual, couple, relationship, and family therapy, including:
- EMDR
- Family therapy
- Play and art therapy
- Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)
Collaborate Intensive Bridging Services (CIBS)
 
Provides a combination of intensive in-home therapy and a brief placement in an intensive residential treatment center for youth ages 8 - 17, and is designed to help kids who struggle with:
- Aggression/fighting
- Self-harm
- Depression
- Truancy
- Acting out in-home or school
- Theft
Early Childhood Therapy
 
Provides early childhood therapy for young children ages birth - 5. Specializing in supporting small children and their families who struggle with emotional and behavioral disturbances caused by mental health disorders, neurological vulnerabilities, and traumatic experiences, including abuse, neglect, exposure to domestic violence, exposure to drugs or alcohol, disrupted attachments to adults, and significant changes in the family. This program includes a range of services and treatments that can be provided in the office, in-home, or via telehealth, including:
- Outpatient therapy
- Parent-child therapy
- Parent support groups
- PASS Day Treatment Program
- Trauma Informed Play Therapy
PASS Day Treatment
 
Provides a day treatment program for young children ages 3 - 5 who need extra support. Provides consistent, ongoing care, every Monday - Friday for young children who are struggling with: 
- Calming down
- Fear
- Following directions
- Talking back
- Throwing temper tantrums
- And/or general unhappiness
Transportation is provided for children who are eligible for the program.
 
Intensive Home-Based Services
 
Provides intensive home-based services for children and youth ages birth - 17 including intensive, in-home, family-focused services for kids ages 8 - 17 who have significant emotional and behavioral problems. Our treatment approach is called Systemic Family Therapy (SFT), which helps address depression, anxiety, oppositional behavior, abuse, trauma, delinquency, and more.
 
Also provides intensive treatment for children ages birth - 17 who are in foster care. Services are provided for both the children as well as their foster parents and birth/adoptive families.
Psychological Testing
Provides comprehensive and individualized psychological testing for youth ages 6 - 18 and adults. Testing includes a wide variety of psychological assessments, such as diagnostic evaluations, ADHD assessment, personality, and cognitive evaluations.

Provides alternate dispute resolution services, including conflict coaching, mediation, meeting and restorative services and conflict resolution training. Services are provided by staff and trained volunteers who are residents of the community. Sessions are scheduled at a time and location convenient to parties including night and weekend hours.
Dispute resolution services are available for a wide range of disputes including but not limited to:
- Family conflicts
- Elder care issues
- Juvenile restorative justice diversion programming
- Neighbor conflicts
- Landlord/tenant disputes, pre-file eviction
- Unmarried couple separation
- Post divorce and parenting time
- Roommate disagreements
- Employer/employee
- Business/consumer
- Conflict between students in school
- Conflict within neighborhood/community groups, boards of directors, businesses, condo-townhouse associations and co-ops

EmPATH Unit and Behavioral Health Services - CentraCare - Saint Cloud Hospital
Provides an Emergency Psychiatric Assessment, Treatment, and Healing Unit (EmPATH Unit) and behavioral health services to people of all ages experiencing problems with addiction or other mental health issues. Services include:
- A care model providing the EmPATH Unit to treat patients with emergency mental health needs. The unit transitions patients from the emergency department into a welcoming and calm setting to provide assessment, initial treatment, and begin the healing process. A treatment plan is developed prior to transitioning to the next level of recommended care.
- Addressing a wide variety of challenges such as abuse/neglect, addictions, ADHD, adjustment problems, anxiety, autism, behavior problems, chronic medical conditions, co-dependency, depression, divorce issues, eating disorders, family changes, family dysfunction, grief/loss issues, learning disorders, parenting issues, personality disorders, PTSD, recovery/relapse issues, relationship issues, school problems, and women's issues.
- Comprehensive inpatient services for complete psychiatric care and treatment plan.
- Outpatient services such as individual, couples, and family therapy.
- Psychological testing for diagnosis and personalized treatment planning.
- Treatment may include educational programs on wellness and relationships, group therapy, leisure and recreational therapy, comprehensive evaluation, medications, and psychiatric psychotherapy.
