The U-Go Free Summer Youth Pass allows children to have unlimited rides on the Metro Bus Fixed Route System from June 1 - August 31 (for the year it is issued).

• An optional bus riding skills class is offered through the Metro Bus Mobility Training Center
• Summer youth groups may obtain quantities of passes for summer activities and transportation

Medicare Savings Programs have four sub-programs: ?Qualified Disabled and Working Individuals (QDWI): Helps pay for Part A premiums only. Qualifying Individual (QI): Helps pay for Part B premiums only.? Qualified Medicare Beneficiary (QMB): Helps pay for Part A and B premiums as well as deductibles, coinsurance, and copayments. Specified Low-Income Medicare Beneficiary (SLMB): Helps pay for Part B premiums only.? Monthly income and countable resource limits apply. Countable resources include any checking or savings funds, stocks, and bonds. A home, one car, a burial plot, $1500 in burial expenses (if the amount is set aside), furniture, and other household items are not applied toward the resource limit. Participants pay for Medicare services that aren't covered, such as routine physicals and prescription drugs.? ?Generally, under these programs, premiums are put back into the monthly Social Security check, increasing participants' monthly income.

• Individualized assessment: Care managers perform a comprehensive, multi-dimensional assessment which evaluates memory and cognition, health, daily functioning, safety, environment, support systems, and future planning

• Liaison: Provides a communication resource and liaison for family members who cannot be present to keep them informed and involved

• On-going consultation: Care managers can meet with family members to review the individual's situation and provide consultation regarding services, housing, care options, and other concerns.

• Personal care plan: Care managers develop a tailored care plan which includes recommendations for services necessary to remain living safely at home, options available for other living arrangements, financial and legal planning tools needed to address vulnerabilities, problem-solving strategies, and other services and supports needed to enhance safety and quality of life.

• Service coordination and monitoring: Care managers interact with health care and other professionals to ensure high quality, cost-effective services and care are provided. Our care managers monitor these supports to ensure needs are met on an ongoing basis. As care needs change, our care managers reassess and make recommendations about how to adjust the care plan to make certain that the individual's needs continue to be met.

Offers certified Child Passenger Safety Technicians to assist caregivers with car seat installation and education on Minnesota's Child Passenger Restraint Law. Technicians provide guidance on proper use and fit for rear-facing, forward-facing, and booster seats, as well as seat belts. Free car seats are also available for qualifying low-income families or those on Medical Assistance.

Financial counselors can help individuals understand patients' responsibilities and may offer no-interest payment plans. Other options may include:

• Discount Program: Contracts with Change Healthcare to assist patients with Minnesota Healthcare Program or Hennepin Healthcare Charity Care applications. Individuals who are uninsured or underinsured and have incomes less than 250% of the Federal Poverty Guideline may be eligible for getting help to pay their medical bill

• MNsure Enrollment: On the MNsure marketplace, individuals can find, compare, and choose healthcare coverage, including commercial health insurance plans and low-cost or free plans through government programs. A financial counselor can help complete the online application. If eligible for a government program such as Medical Assistance or Minnesota Care, individuals may enroll through MNsure year-round.

The assessment helps determine public programs that might pay for an individual's long-term services and supports or help them learn about other options if they don't qualify for publicly funded programs. It is required to determine eligibility for: Community First Services and Supports - CFSS (formerly known as Personal Care Assistance - PCA) Home and Community-Based Services Waivers based on age:

Intellectual and Developmental Disability (IDD) Services (also known as Rule 185 Case Management) Family, friends, or others can attend the assessment. The assessment is in person at the individual's home or at a place of their choice, and it lasts about two hours.

ECFE is a parenting education program for all Minnesota families with children between the ages of birth to Kindergarten entrance. Learning opportunities through ECFE may occur in a variety of settings, such as the families' homes, district or community sites, and online parenting education and support. Programming and education services often include parents/caregivers and children but may just include parents/caregivers. Works to strengthen families and engage the ability of all parents to provide an environment that supports the healthy development of their child. Services may include:

Mental health professionals and practitioners provide counseling to families in their homes. Through a variety of programs and interventions, families identify their strengths, explore concerns, and create and work an action plan to address challenges.
