• Outpatient treatment

• Chemical health assessment

• Determine eligibility for Behavioral Health Fund

Down Payment Assistance Program This is only for existing buildings, not for a person building a new home. It also does not apply to mobile homes. The home must be within the city limits of Gaylord. The economic authority accepts applications on a monthly basis and the board approves them once a month. Business Loans: Revolving loan fund information for homes and businesses. The Economic Development Authority (EDA) advises the city council regarding economic development, housing, and redevelopment matters, and exercises certain powers as authorized by the council.

Offers comprehensive behavioral health services, including diagnostic assessments, education, individual therapy, crisis intervention, care coordination, and advocacy. Services are person-centered, culturally appropriate, and trauma-informed. Partners with local schools to provide mental health support and substance use services where available. Some services may be provided in person, by phone, or via video, depending on provider availability.

Provides nutrition education and special foods which promote good health for pregnant, breastfeeding and postpartum women, infants, and/or children up to age five Services include:

• Breastfeeding education

• Health assessment including height, weight, and hemoglobin is performed

• Referral to health care and other services

• Vouchers given to participants to purchase nutritious foods

Medicare Savings Programs have four sub-programs: Qualified Disabled and Working Individuals (QDWI): Helps pay for Part A premiums only. Qualifying Individual (QI): Helps pay for Part B premiums only. Qualified Medicare Beneficiary (QMB): Helps pay for Part A and B premiums as well as deductibles, coinsurance, and copayments. Specified Low-Income Medicare Beneficiary (SLMB): Helps pay for Part B premiums only. ?Monthly income and countable resource limits apply. Countable resources include any checking or savings funds, stocks, and bonds. A home, one car, a burial plot, $1500 in burial expenses (if the amount is set aside), furniture, and other household items are not applied toward the resource limit. Participants pay for Medicare services that aren't covered, such as routine physicals and prescription drugs.? ?Generally under these programs, premiums are put back into the monthly Social Security check, increasing participants' monthly income.

Income eligible older adults ages 60 or older may receive up to $50 worth of food once a month. Staff can assist with sign-up, delivery, or pick up of food boxes.

• Syringe exchange programs
