St Charles Assisted Living is an assisted living facility serving adults, ages 55 and older. Amenities include:

• Emergency funds may be available if the individual cannot pay for basic needs, such as shelter or food, and their health or safety is at risk because of this

People who get GA are also eligible for help with medical and food costs through Medical Assistance - MA and the Supplemental Nutrition Assistance Program - SNAP.

Provides investigation and intervention services for child abuse, neglect, and/or abandonment. Services include:

• Accepts reports of abuse

• Assessment of the initial referral

• Consultation to individuals who suspect abuse

• Consultation with the family to determine the nature of the problem

• Explanation of reporting statutes and answer questions regarding agency practices

Provides an intensive outpatient program for alcohol and drug recovery to both men and women ages 16 years and older at all locations. The program lasts eight weeks and offers daytime and evening sessions depending on the location. The program includes:

• Integrated drug and alcohol education, as well as the Family Program into the treatment plan

• Group therapy sessions lectures, individual counseling, family education, the 12-step support group program, and a Relapse Prevention Program

• Person-centered approach and followed by continuing care programs

• Weekly treatment planning and individual counseling sessions

Provides education, assistance, and leadership opportunities to youth in the long-term foster care system and those transitioning to independent living.

• Augmented and Alternative Communication Clinic: Multidisciplinary assistance to provide communication systems for patients who otherwise are unable to communicate effectively

• Craniofacial and plastic surgery

• Feeding Clinic: Provides evaluation of children and treatment of difficulty in chewing and swallowing

• Neurology: Manage medical treatment of disorders of the nervous system such as the brain, spinal cord, and nerves

• Orthopedics: Treats routine and complex conditions that affect bones, muscles, tendons, and ligaments

• Orthotics, Prosthetics, and Seating: Assistive technology team designs customized braces, prostheses, and other adaptive technology. Seating evaluation clinic for wheelchairs and other adaptive equipment

• Pediatric Sports Medicine: Treat sports-related or soft tissue injuries

• Pediatric and General Medicine: Treats a wide range of conditions which include digestive issues, endocrine issues, infectious diseases, nutrition and feeding, and respiratory needs

Patients who need to speak with a financial counselor, need cost of care estimates, or determine if they may qualify for financial assistance to pay for procedures, may reach out about options. Financial counselors can help individuals understand what patients' responsibilities will be, and may offer no interest payment plans. Other options may include:

• Discount Program: Individuals who are uninsured or underinsured and have incomes less than 250% of the Federal Poverty Guideline may be eligible for help to pay their medical bills

• Elevate Patient Financial Solutions: Help patients without medical insurance find resources to help pay for medical bills

Provides cash assistance to help adults receiving Supplemental Security Income - SSI pay for their basic needs. Adults who are blind, have a disability, or are age 65 and older but don't qualify for SSI because of their income may still be eligible for MSA if they meet income requirements. The basic monthly MSA grant is calculated using either the Maximum SSI amount (Federal Benefit Rate) for the adult or their countable income (if they do not receive SSI), and the MSA standard set by state law. Special Needs Grant MSA also may provide special payments for adults who need:

• Certain home repairs

• Certain household furniture and appliances

• Guardian or conservator service fees

• Help paying for housing costs through MSA Housing Assistance

• Medically prescribed special diets

• Representative payee services

Adults who receive MSA are also eligible for help with medical, food, and housing costs through Medical Assistance - MA, the Supplemental Nutrition Assistance Program - SNAP, and MSA Housing Assistance programs.

??T?he FAIM program provides working low-income Minnesotans to increase their savings, build assets, and enter the financial mainstream. The program is a matched savings project to help participants purchase a first home, pursue higher education, launch a small business, or purchase a vehicle. Participants in the program deposit up to $84 monthly from their earned income into a Family Asset Account (custodial savings account). Participants have up to 24 months to save their earnings and may be matched by the program rate of 3:1 upon successful completion of their contract. All participants are also required to complete a 12-hour Financial Literacy course and 10 hours of asset-specific training.

Pays for room and board for seniors and adults with disabilities who have low incomes. Aims to reduce and prevent people from living in institutions or becoming homeless. All recipients receive assistance each month to help pay for rent and food. Some recipients also receive Housing Support supplemental funding to provide other services if they don't qualify for home and community-based waiver programs, including but not limited to:

• Arranging for medical and social services

• Arranging for meetings and appointments

• Assistance with transportation

• Medication reminders

The individual's housing provider receives payments to pay for rent, utilities, food, household supplies, and other necessities.
