Project coordinators are certified accessibility specialists who will perform an environmental accessibility assessment of a home to help identify areas that could be improved for mobility and safety.Specialists will look at the construction and layout of the home as well as how things are used. They examine flooring, hallways, doorways, bathrooms, and kitchens to determine how accessible they are. Coordinators will determine plans that can help keep people in their homes longer and recommend any additional assistive technology options that would be beneficial.

Provides support to General Relief recipients, including those receiving county-funded cash grants and emergency assistance, by addressing disputes and complaints through mediation, negotiation, education, or referrals to appropriate agencies or legal services. Offers guidance on the complaint and appeal processes available through health plans and the state. Educates consumers about their rights, relevant laws, regulations, and health care financing through written materials, consultations, or public speaking. Advocates for legislative and systemic reforms in health care and social services to better meet consumer needs.

Direct Access allows an individual to choose a provider to receive comprehensive assessments and access care immediately. The county will assess individuals who need help paying for a chemical use assessment or treatment services through the Behavioral Health Fund. Also provides:- 1:1 and family chemical dependency counseling- Case management and advocacy- Provides referrals to local community service providers for chemical assessments, treatment services in outpatient, inpatient, extended inpatient treatment, halfway house care, and chemical dependency education and prevention programs- Referrals to low-cost community services for client needs beyond the primary chemical problem

Pays for room and board for seniors and adults with disabilities who have low incomes. Aims to reduce and prevent people from living in institutions or becoming homeless.All recipients receive assistance each month to help pay for rent and food. Some recipients also receive Housing Support supplemental funding to provide other services if they don't qualify for home and community-based waiver programs, including but not limited to:- Arranging for medical and social services- Arranging for meetings and appointments- Assistance with transportation- Medication remindersThe individual's housing provider receives payments to pay for rent, utilities, food, household supplies, and other necessities.
