- 24-Hour Responder Service: On-call staff help with health and personal injuries. The service is often paired with Independent Home Support with Training program. Staff will respond by phone or in person depending on the level of services. - Acute Behavioral Services for Children and Adults: Services provided to individuals with interfering behaviors and/or anti-social issues, who have a history of law enforcement involvement, have difficulty living with others and require high staffing needs, are in frequent crisis situations, or coming out of an institution. - Case Management: Available to residents of Hennepin, Ramsey, and Wright Counties who receive funding through various waivers and/or programs. Assists navigating the social service system and provides information and advocacy. - Integrated Community Supports: Provides support and training/habilitation in community living service categories to adults ages 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person's living unit or in the community. - Individualized Home Supports: Provides a blend of in-person and remote services that focus on support and training of a person who needs to maintain or increase independence for their continued experience living in the community. Also offers services to relatives working to support the person's goals. - Residential Services: Operates residences for adults with significant behavioral, medical, or physical challenges including brain injuries. Residential group home programs are licensed as adult or child foster care, or a supervised living facility. Each site serves 2 - 6 people. - Supportive Employment Services: Provides individuals with intellectual and developmental disabilities assistance with initial stages of the employment assessment period, job search and development, job coaching, and working independently - Vocational Rehabilitation Services: Offers a variety of services including job-seeking skills training, job coaching, social coaching, post-secondary education, and more. City bus service that provides an on-demand system door to door. Individuals call to state where they are, where they want to go, when they want to get there, and when they want to return. All buses have a wheelchair lift and space for tying down either 3 or 5 wheelchairs.

- Provides local transportation and/or Dial-A-Ride services for all residents of Cannon Falls/Randolph, Faribault, Northfield, Kenyon/Wanamingo, Lake City/Frontenac, Lonsdale, Pine Island, Plainview/Elgin, Red Wing, Wabasha/Kellogg, and Zumbrota/Mazeppa. - In some cities, Hiawathaland Transit has established routes that serve the majority of the city limits. Riders may get on the route(s) at their own discretion; pre-scheduled route deviations are also available. - In other communities, Dial-A-Ride bus is available. - In cities where both route and Dial-A-Ride exist, some eligibility restrictions do apply to the Dial-A-Ride service. Each bus is handicap accessible. Assistants accompanying a person with disabilities ride free. Drivers do not assist individuals into homes, apartment buildings, or businesses. Tokens are encouraged. Drivers do not make change. Passengers are required to pay their fare before their ride and will be charged for any no-call no-shows. Drivers do not assist passengers with packages and number of packages may be limited to what a passenger can independently carry and can be properly and safely secured. Twenty-four hour advance notice is encouraged for any ride needing the use of the lift.

Services include: - In-home respite: Service is provided to a person in their home or the family home. - Out-of-home respite: Service is provided to a person in a licensed or unlicensed setting that is not the person's home. - Respite level A: Active, out in the community - Respite level B: Stay at home and local outings only - Respite level C: Sleep in - Respite level D: 24-hour period Homemaker Services - Services that help a person manage general cleaning and household activities. Services include: - Cleaning: light housekeeping tasks, laundry services - Home management: cleaning, transportation arrangement, meal preparation, shopping for food, and household items - Assistance with ADLs: cleaning, assistance with ADLs (ambulating, bathing, dressing, eating, grooming, toileting) *Training is not covered under this service. To receive homemaker services, homemaker cleaning must be the primary service provided. County-Funded Semi-Independent Living Services (SILS) - Services needed to maintain and improve an adult's capacity to live in the community. Supports people in ways that enable them to achieve personally desired outcomes and lead self-directed lives including: - Money management - Meal preparation - Shopping - Obtaining and maintaining a home - Self administration of medication - Formal skill building

The Hamline Midway Elders Living At Home Block Nurse Program provides or arranges support for elder neighbors to assist them in living at home as long as possible. This support, which may otherwise not be available, uses the program's small staff, volunteers and/or paid nursing staff. Services include: - Caregiver coaching and support groups - Community Gardening - Door-to-door transportation, both assisted and non-assisted, provided by volunteers. There is no charge, but donations are accepted. Accessible vehicles can be requested. - Elders resource list of meal and grocery delivery, handyman services, modifications, etc. available by phone or Internet - Exercise classes - Friendly visiting - Health education classes and events - In-home eldercare assessment and resource recommendation - In-home Internet/computer tutorial - Linkage to county services - Nursing referrals - Phone visiting - Respite care - Seasonal raking and snow shoveling - Shopping assistance up to twice a month - Spring/Fall yard clean-ups - Volunteer opportunities
