Office of Health Facility Complaints - Minnesota Department of Health

Serves the general public by investigating reports of health care facilities violating state or federal regulations. Addresses complaints, questions, or concerns related to licensed facilities: hospitals, nursing homes, boarding care homes, supervised living facilities, assisted living, and home health agencies. This office cannot investigate billing or insurance concerns, medical clinics, or personal care attendants who do not work for a home care agency. Though if an individual is unsure where to file their complaint, contact the office.

DHS Licensing Division - Minnesota Department of Human Services

Protects the health, safety, and rights of individuals receiving services by ensuring providers meet minimum standards of care and physical environment. The program addresses complaints related to child care and children's residential facilities, including issues of licensing, cleanliness, safety, treatment of residents, quality of care, fees, and conduct of personnel. It also involves the licensing and regulation of child care providers, children's group homes, and foster homes, including reviewing applications, issuing or denying licenses, inspecting facilities for compliance, and taking disciplinary action for noncompliance. Investigate licensing violations and possible maltreatment at: - Adult day care centers - Child care centers - Children's residential facilities - Home and community-based services for people who are age 65 or older or have disabilities - Licensed private agencies - Mental health programs - Residential and outpatient services for people with substance use disorders

Ombudsman for Long-Term Care - Minnesota Board on Aging

An independent state agency that works to preserve individual rights for all who are living in a long-term care community. Serves individuals needing or receiving long-term care through complaint investigation, advocacy, and education. Services provided: - Advocates for reform through state and federal legislation and the health care and social services system to better meet consumer needs - Investigates complaints and works to resolve disputes between consumers and nursing homes, boarding care homes, adult foster care, housing with services/assisted living residences, home care agencies, hospitals and government agencies through mediation, negotiation, education or referral to appropriate state or federal enforcement agencies or legal services - Provides information and education about consumers' rights, laws and regulations, and the financing of health care and long-term care services through written materials, individual consultation, or public speaking
