MnCHOICES - Polk County Social Services

A comprehensive web-based application that integrates assessment and support planning for people who need long-term services and supports in Minnesota. The assessment takes a person-centered approach to ensure that services meet each individual's strengths, goals, preferences, and assessed needs. The assessment replaces the following assessment tools: Developmental Disability Screening Long-term Care Consultation Assessment Personal Care Assistance Assessment Home Care Nursing Assessment (to be included in a future enhancement) The assessment determines eligibility for publicly funded programs and services including: Alternative Care Program Brain Injury Waiver Community Alternative Care Waiver Community Alternatives for Disabled Individuals Waiver Developmental Disability Waiver Elderly Waiver Case management for people with developmental disabilities Semi-Independent Living Services NOTE: Community or in-person screening is required in some situations to determine eligibility for waiver services
